When a family enrolls with CCCC, we do not assign one care coordinator and call it done. We build a Child-Family Team.

That team is made up of the people who matter most to your child, including family members, teachers, therapists, coaches, faith leaders, and anyone who plays a role in their life. Then we bring that team together around a single Individualized Service Plan.

Because no one person has every answer. But a team? A team can change everything. That is what wraparound care actually means.
