"What happens if I become unable to make my own financial or medical decisions?"

If you haven't set up a Power-of-Attorney (POA), the court may step in and choose someone to act on your behalf. Setting up a POA in advance is the simplest way to maintain control over your legal, medical, and financial affairs before a disability or crisis occurs.

Through Guidance Carolina's RISE for Change Program, we are expanding our capacity to protect, empower, and advocate for vulnerable community members across North Carolina. Over the next three years, RISE for Change will:

βœ… Add 3 full-time Elder Justice Care Managers to serve over 640 high-risk older adults.
βœ… Deliver trauma-informed, client-led care tailored to unique individual needs.
βœ… Strengthen cross-sector collaboration with law enforcement, healthcare providers, legal aid, APS, and local partners.
βœ… Develop personalized care plans that ensure safety, housing stability, healthcare access, and financial well-being.

Guidance Carolina was happy to present "Pathways to Adulthood" at Peacehaven last month.

We had a great informative session; answering lots of questions, having deeper conversations about what it means to transition into adulthood for our special needs communities.
