Care Resources  PACE For most people, there are little to no deductibles or co-pays. Participants who do not meet PACE eligibility guidelines can pay for services privately.

Care Resources is a Program of All-inclusive Care for the Elderly, or PACE®, that provides wrap-around supportive services for people 55 years and older to keep them healthy and living independently in their homes. We partner with participants and their loved ones to develop a comprehensive care plan that meets the physical, mental, emotional and social needs of each participant and keeps them out

of nursing homes. Through our day centers in Grand Rapids and Lowell, we offer:
• Primary health care
• Home health care
• Clinic
• Prescription medication
• Rehabilitation services
• Activities
• Nutrition services
• Social services
• Transportation

PACE uses Medicare and Medicaid funds to cover all medically necessary care and services. Care Resources PACE serves those who are 55 and older and are:
• Residents of Kent County

Physical therapy through Care Resources is special because rather than an isolated service, it is part of a team-based approach to support every aspect of health. Physical therapists communicate daily with doctors, nurses, social workers and more, providing coordinated care at our day center or at home with no separate insurance approvals or hidden co-pays.

A PACE pilot program to advocate and care for older adults launched in 1995, but it would be another 20 years before the official creation of the PACE Association of Michigan. Care Resources was one of six original members, alongside programs in Muskegon, Battle Creek and Detroit that had already been collaborating, sharing best practices and working tirelessly to secure funding for programs.

Today, more than 85% of Michigan’s older adults have access to PACE programs thanks to the association’s efforts with the Michigan Legislature and Michigan Department of Health and Human Services. We remain actively involved to protect and grow PACE’s footprint in the state.

This summer, we strengthened participant care at our Lowell center through our ongoing partnership with the Dementia Institute.

Over four weekly, two-hour sessions, nine more team members became dementia-certified professionals, meaning our entire team at this location has now completed this specialized training! A variety of departments participated — everything from physical therapy and social work to nutrition services and admin support. Because dementia touches every aspect of care, every role has an important part to play.

We're incredibly grateful to the Lowell Area Chamber of Commerce for making this training possible through the Ivan K. Blough Training Fund. With your support, we can continue to provide compassionate, knowledgeable care and give family caregivers reassurance and respite. Thank you for recognizing the importance of dementia education and helping us continue to advance the care our community deserves!
