Sandy is one of the many individuals Thresholds has he honor of supporting through our community-based OBRA case management program. On a monthly basis, Sandy meets with her case manager to assess her needs and support her mental health while residing in a nursing facility.
Sandy doesn’t get out into the community as often as she once did, which makes visits from others especially meaningful to her. Recently, Sandy was visited by Thresholds’ Life Skills Hospitality Group, a wonderful group of individuals who visit nursing facilities to spend time socializing and connecting with the people we serve through case management. These visits provide companionship and opportunities for social connection, with peers participating alongside the guidance and support of our Life Skills coaches.
Their visit truly brightens Sandy’s day! One of her peers even brought her a special tiara because Sandy knew she wouldn’t be able to attend the upcoming Thresholds Fall Ball in October. So, instead of attending the fall ball, Sandy can wear her tiara, look fancy, and feel fancy every day!
We are grateful for the friendships, kindness, and meaningful connections that make moments like this so special.
