Oaklawn PATH (Projects for Assistance in the Transition from Homelessness) they assist individuals and fmailies who are homeless or at rick of homelessness and have mental illness.
Service include:
*** Access to Mental Health and Addiction services
*** Assistance in finding emergency shelter and affordable housing
*** Assistance in obtaining health insurance
*** Short-term case management

Oaklawn PATH (Projects for Assisting in the Transition from Homelessness) provides services to the homeless with mental illness and addictions in Elkhart County enrolling approximately 200 people annually. Services include facilitating access to mental health and addiction services, primary care physicians, housing, and benefits (SNAP, medicaid, etc). We work closely with and collaborate with other community partners who also provide services to the homeless.
