AHCM
The Accountable Health Communities Model addresses a critical gap between clinical care and community services in the current health care delivery system by testing whether systematically identifying and addressing the health-related social needs of Medicare and Medicaid beneficiaries’ through screening, referral, and community navigation services will impact health care costs and reduce health care utilization. Built for Zero
West Mountain Regional Health Alliance is the Community and Data Lead for Built for Zero, a national initiative that works to functionally end homelessness. Communities achieve this through collecting real-time data on individuals experiencing homelessness and connecting them with needed services. At the same time, communities are creating coalitions of many organizations to bolster current resources and develop new ones to ensure a strong range of programs and services to solve homelessness and prevent future homelessness. The program has been successful in a variety of settings, and WMRHA is excited to partner with several local organizations to bring BFZ to this region and seeing it through until every chronically homeless individual is housed. PSH
Permanent supportive housing (PSH) is an intervention that combines affordable housing assistance with voluntary support services to address the needs of those who have experienced chronic homelessness. Services are designed to build independent living with tenancy skills and connect people with community-based health care, treatment and employment services. Partnering with the Colorado Health Foundation, West Mountain Regional Health Alliance is currently developing an implementation plan to bring Permanent Supportive Housing to our valley. Connects primary care with community-based behavioral health and social services. Community Resource Network
Partners with clinical quality improvement teams to help practices prepare for new models of care and reach their goals. Recommends reliable resources to improve health outcomes. Thriving Colorado Dashboard
The Thriving Colorado Dashboard is a statewide community health data dashboard. This dashboard will allow Colorado to adopt one common tool to align efforts and strategies with agencies and organizations across the state. The Thriving Colorado Dashboard will support collective impact efforts toward our shared goal of making Colorado the healthiest state in the nation. About WMRHA
West Mountain Regional Health Alliance (WMRHA) formed as a regional entity in 2010 to examine the access to care gap of prenatal care for low-income women in western Eagle County. This lead to a collaborative Prenatal Program between Pitkin County and Eagle County. The success of the entity highlighted the need for the group to continue its efforts to identify issues and seek regional solutions. In order to do so, the alliance expanded to a broader community alliance that it is today. Today the WMRHA consists of members from many sectors of the healthcare industry, including: healthcare organizations, public health, human services, clinical providers, schools, private sector, and community organizations. All of these groups are committed to creating an integrated healthcare system (physical, behavioral and oral health) in Eagle, Garfield, Pitkin and Summit counties. The Alliance creates a neutral space for "multi-sector and multi-stakeholders" to convene, identify, understand, and work together on regional issues to create solutions. WMRHA works in collaboration with the members to address large, system-level healthcare challenges through alignment of initiatives and efforts and advocates for access to care.

We’re helping spread the word about Valley View’s Celebration of Survivorship, honoring cancer survivors and the loved ones who support them.

Join cancer survivors from across the community and members of the Calaway•Young Cancer Center team for an evening of live music, light bites, reflection, and recognition of survivors past and present.

Our 2025 Annual Report is here!

This year marks an exciting milestone for West Mountain Regional Health Alliance—10 years of bringing partners together to improve health and well-being across Eagle, Garfield, and Pitkin counties.

From advancing housing stability through the Valley Alliance to End Homelessness, to strengthening food security, expanding access to community resources, and supporting regional collaboration, our work is made possible by the incredible partners, funders, and community members who share our vision for a healthier region.

In 2025, we continued to invest in solutions that address the root causes of health challenges while building stronger systems that support everyone in our communities.
Thank you to all who have been part of this journey. We look forward to continuing this work together in the years ahead.

Finding help just got easier for residents across Pitkin, Garfield, and Eagle counties.

After two years of development, the West Mountain Regional Health Alliance has launched a new tri-county resource directory designed to connect people with essential services throughout our region. The directory includes organizations that provide legal assistance, support survivors of domestic violence, operate animal shelters, offer health and social services, and much more.

Resource directories have long been valuable tools for healthcare professionals and social workers, helping connect individuals and families with the support they need. Now, this comprehensive local resource is available to strengthen those connections across our communities.

Aspen Public Radio’s Regan Mertz recently spoke with our Kaen Lapides, Regional Health Connector, about the new directory and its impact on the region.

Colorado’s State of Homelessness in Colorado 2025 report offers a comprehensive look at statewide trends in housing instability, shelter utilization, demographic shifts, and system performance. It serves as a key resource for policymakers, service providers, healthcare organizations, housing agencies, and community partners working to address homelessness across the state.

We are proud to highlight Karina Rasmussen, Housing Access Data Coordinator at West Mountain Regional Health Alliance, for her contribution to the report. Her work in housing access data coordination, data quality, and regional collaboration helped ensure that local and regional insights were accurately represented in the statewide analysis.

The report also underscores ongoing challenges, including limited affordable housing, rising housing costs, behavioral health needs, and disparities affecting vulnerable populations—while reinforcing the importance of coordinated, data-informed regional responses.

West Mountain Regional Health Alliance’s April Quarterly Meeting brought together partners from across Eagle, Garfield, and Pitkin counties to share updates, reflect on regional progress, and discuss ongoing community needs.

This year also marked 10 years of the Alliance, highlighting a decade of collaboration focused on strengthening health and wellbeing across the mountain region.

Meeting discussions included regional housing initiatives, food security strategies, healthcare access data, behavioral health gaps, and community support resources. Partners also shared updates on investments supporting local agencies and ongoing efforts to improve accessibility and long-term sustainability across the region.

Thank you to all presenters, partners, and community members who participated in this quarter’s conversation and continue advancing this work together.

At West Mountain Regional Health Alliance, hearing directly from our partners is essential to understanding the real impact of our work. We’re grateful for leaders like Dustin Moyer, CEO of Mountain Family Health Centers, who continue to share how WMRHA’s focus on Social Determinants of Health (SDOH) helps fill the gaps in community members’ health outside of clinic walls and improve access to care across tri-county region. Last Monday we had our quarterly meeting! If you missed it, check this link out for the full video.

The West Mountain Regional Health Alliance (WMRHA) brought together partners from Eagle, Garfield, and Pitkin counties to collaborate on advancing shared health priorities through data, policy, and community action.

This session highlighted how local and statewide data can drive stronger decision-making and advocacy, along with updates on our Theory of Change, membership model, and regional insights on health access and housing.

We also heard a powerful presentation featuring new legislative district profiles based on 2025 survey data—helping us better understand community needs and strengthen engagement with policymakers.

Plus, regional partners shared updates on financial empowerment and community education efforts making a real impact on the ground.
