Tip of the day: Every adult should have a Healthcare Proxy.
What is a health care proxy? A health care proxy is a document that names someone you trust as your proxy, or agent, to express your wishes and make health care decisions for you if you are unable to speak for yourself.
Why do I need a health care proxy?
A health care proxy allows you to appoint someone else to act as your agent for medical decisions. It will ensure that your medical treatment instructions are carried out, and it is especially important to have a health care proxy if you and your family may disagree about treatment.
Assumption: Health care proxies are only for those who are sick or elderly. NOT true. Everyone needs a health care proxy, not just the elderly. Anybody can be in a situation where they're temporarily unable to speak for themselves. By naming someone in a health care proxy to speak for you and by informing them of your wishes, you relieve the potential burden on others.

1. Choose someone you trust the most to:
2. Print a Health Care Proxy (may vary by state)
3. Complete and sign the form
4. Ask two other people to sign the form as witnesses
5. Put the form where it is easy to find
6. Give a copy of your health care proxy form to your health care agent
7. Remember to talk about your medical care preferences with your health care agent
