October is Breast Cancer Awareness Month. Did you know that more than 1 in 5 North Carolina women ages 40-74 reported not having a mammogram in the past 2 years?

Primary care teams can help close that gap by helping patients understand when breast cancer screening may be appropriate based on their age, health history, and individual risk factors. Now is a good time to review patient lists, identify patients who may be due for screening, and build that important outreach into existing workflows.

For small, independent practices, improving patient care often starts with having the right information and a team prepared to act on it.

At Faith Family Medical, a two-provider practice, the team uses the CCNC patient one-pager before visits to identify open care gaps, then relies on a highly cross-trained staff to help coordinate screenings, referrals, and follow up.

That team-based approach helped the practice exceed year-end benchmarks for cervical cancer screening, breast cancer screening, and adult ambulatory care visits ahead of schedule.

Looking at the bigger picture can help teams make the most of those moments – whether that means helping a patient schedule a routine visit, catching up on a recommended screening, or addressing another health need.

A routine visit can be an important opportunity to talk about colorectal cancer screening.

American Cancer Society research found that among adults who were overdue for colorectal cancer screening and had a wellness visit in the past year, only about 27% reported receiving a screening recommendation from their clinician.

Practices can help close that communication gap by identifying patients who may be overdue, using point-of-care reminders where available, and making screening part of the conversation during routine visits.
