After being recognized at the CCPN Annual Clinician Conference in May, the team recently gathered for a special lunch to celebrate the achievement together.

It was a well-deserved celebration of the teamwork and dedication that go into providing high-quality care for children and families every day.

Respiratory virus season often arrives quickly. Now is a good time for primary care providers to talk with patients about recommended vaccines, preventive care, and individual health concerns that may affect their risk.

Late summer is also an opportunity to review vaccine workflows, prepare patient outreach, and ensure the practice is ready for the season ahead.

Meet “Mobile Mable,” Hodges Family Practice’s mobile healthcare unit bringing preventive care and wellness services directly to patients in its rural community.

The practice launched Mobile Mable this spring after receiving a grant from the Office of Rural Health. By partnering with schools, daycares, housing authorities, shelters, and community organizations, the team is helping patients access care where they live, learn, and work.

Mobile Mable has helped the practice reach patients who may have difficulty traveling to the office, increase participation in wellness visits, and connect more community members with needed care. CCPN quality bonus funding also helps support maintenance, gas, and staffing costs.

Congratulations to Marie Polanco and the entire Hodges Family Practice team for finding a creative way to bring care closer to the patients and communities they serve.

A 2025 systematic review and meta-analysis published in JAMA Network Open found that outpatient follow-up after hospital discharge is associated with a lower risk of 30-day readmissions.

CCNC's care managers partner with practices to coordinate follow-up, address barriers, and help patients reconnect with their team, supporting safer transitions and better outcomes.
