Community Case Manager
(Part-Time, 15 hrs/week)

Role Summary
The Community Case Manager provides client-centered case management, connects households to resources, and advances long-term housing goals. The role supports Community Services, outreach, and program operations to ensure seamless delivery of Simon House services.

Key Responsibilities
• Deliver case management for Prevention programs; conduct outreach as needed.
• Research, cultivate, and maintain housing options; assist with placements.
• Support Community Services appointments and client follow-up; maintain pantry organization and distributions.
• Screen/interview clients for services; develop individualized goals and action plans.
• Maintain accurate client records and supportive-services logs.
• Collect data for evaluations, fundraising, and board reports.
• Participate in internal/external trainings and all-staff meetings; assist with social media and community awareness.
• Submit ROSM applications for clients.
• Provide support to our clients through our social media.
• Attend & represent Simon House at community events.
• Perform other duties as assigned by the Executive Director.

Qualifications
• Knowledge of mental illness, substance use disorder, and homelessness (required).
• Familiarity with Franklin County resources (preferred).
• Strong oral/written communication
• Proven skills in goal setting, prioritizing, meeting deadlines, and handling multiple needs.
• Working knowledge of case management practices and documentation; proactive problem-solving and sound judgment.
• Collaborative approach with clients, team members, and community partners.
Requirements
• Valid Kentucky driver’s license, reliable vehicle, and current auto insurance.
• Ability to lift 35 lbs and climb stairs.
• Criminal background check required as a condition of employment.

✨ MEET THE BOARD SPOTLIGHT ✨

We are honored to spotlight Dr. Bucher, a dedicated family medicine physician who has faithfully cared for the Frankfort community for over 16 years. From newborns to the elderly, Dr. Bucher has a true passion for providing compassionate care to patients of all ages.

Dr. Bucher graduated from the University of Virginia School of Medicine and completed his Family Medicine Residency at McLeod Family Medicine in Florence, South Carolina. His years of experience, servant heart, and commitment to families make him an incredible addition to the William Joseph Crisis Nursery Advisory Board.

Dr. Bucher is passionate about helping provide a safe, supportive, and caring environment for newborns and children facing difficult family situations. We are so grateful for his leadership, compassion, and willingness to serve our community in such a meaningful way. 💙

We’re here to support families with essential infant care items, including diapers and wipes, available at no cost while supplies last. This event is open to everyone in the community.
