A narrative review compares dual-root Custodiol, cold blood cardioplegia with selective left coronary ostial supplementation, and repeated-dose warm blood protocols for ALCAPA repair. It highlights delivery challenges, collateral flow, and perfusion logistics. The review does not systematically analyse clinical outcomes or establish superiority of any technique.

A narrative review compares dual-root crystalloid, selective cold blood and repeated-dose warm blood approaches for ALCAPA repair. The synthesis highlights perfusion trade-offs without establishing clinical superiority of any technique.

A 17-centre registry analysis of 502 redo mitral operations through right minithoracotomy reports 7.6% in-hospital mortality and 3% conversion to sternotomy. Compared with cardioplegia, ventricular fibrillation was associated with higher adjusted odds of low cardiac output, while beating-heart surgery was associated with higher adjusted mortality.

A 502-patient registry examines outcomes after redo mitral surgery through right minithoracotomy. Cardioplegia had favourable associations, but baseline risk differences and small subgroups limit causal conclusions.

This narrative review examines vasoplegic syndrome after cardiac surgery, linking inflammation, nitric oxide excess, vasopressin deficiency, and renin–angiotensin dysfunction to loss of vascular tone. It discusses multimodal vasopressor therapy and the limitations of adjunctive agents.

This narrative review examines mechanisms and multimodal therapy for vasoplegia after cardiopulmonary bypass. IPA highlights perfusion considerations, evidence gaps and an inconsistency in the reported diagnostic thresholds.

A surgical technique report describes a 10-mm direct aortic Y-graft that provides shared access for cardiopulmonary bypass and Abiomed Impella 5.5 insertion through one aortotomy. The approach aims to simplify the transition to temporary mechanical support. Proposed reductions in bleeding and procedural time remain untested, as the report contains no patient series or comparative outcome data.

A surgical technique combines CPB arterial access and Impella 5.5 insertion through one aortic Y-graft. The report outlines practical considerations, while proposed clinical benefits remain untested.

A UK case report describes a 36-year-old woman with fulminant myocarditis who survived refractory cardiac arrest after ECPR, central biventricular assist-device support, and subsequent DCD heart transplantation. She was discharged neurologically intact after 72 days in hospital.

A woman with fulminant myocarditis survived refractory cardiac arrest through ECPR, BiVAD support and DCD heart transplantation. This case highlights perfusion readiness and ventricular unloading while remaining single-patient evidence.

Seattle Children’s describes modified blood priming in 26 patients weighing 2.0–6.5 kg. Calculated red-cell priming combined with autologous priming reduced crystalloid exposure; mean PRBC use was 52 mL in the prime and 96 mL across bypass. The comparison was with the center’s standard prime, not a controlled patient cohort.

A 26-patient series describes calculated red-cell priming combined with autologous priming to limit donor-blood volume. The findings support feasibility, while variable hematocrit results and limited clinical outcome data warrant caution.

A randomized trial in Nepal compared pulsatile and non-pulsatile cardiopulmonary bypass in elective atrial septal defect closure and mitral valve replacement. Among 77 analyzed patients, ICU stay, hospital stay, creatinine, liver tests, and complications did not differ significantly. Urea was slightly higher with pulsatile flow.

A randomized trial found no significant difference in most clinical outcomes among 77 elective ASD and mitral valve surgery patients. Its small sample and methodological limitations mean the results do not establish equivalence or settle the role of pulsatility.

A single-center study of 194 elective ascending aortic and root operations compared transaxillary access with full and partial sternotomy. After propensity matching, transaxillary surgery required longer bypass time, but early clinical outcomes did not differ significantly from partial sternotomy. No transaxillary patient died in hospital or had a stroke.

A single-center study compared transaxillary access with full and partial sternotomy for ascending aortic and root surgery. Matched early outcomes did not differ significantly, but longer bypass time and selection bias require careful interpretation.

A 23-month-old donor supported on VA-ECMO provided a liver recovered using normothermic regional perfusion. The graft went to a child with maple syrup urine disease, whose native liver then benefited a second child with biliary atresia. Both had good early graft function, although postoperative complications occurred.

A liver recovered from a 23-month-old donor using NRP enabled a domino transplant sequence benefiting two children. This case highlights early success alongside postoperative complications and important evidence limitations.
