Please make all Wille Nell Evans scholarship donations/checks to United Bank c/o My Community Mentor Inc, for Willie Nell Evans Scholarship fund. You can also mail your donations to PO Box 34354 US Hwy 98, 36549.

Paperwork will be provided for all substantial donations.

Eligibility:
•Applicant must be a member of My Community Mentors, Inc.
•Applicant must be a senior
•Applicant must demonstrate involvement within the school and community (50 hours min of Community Service)
•Applicant must show proof of enrollment from the college or university they intend to enroll at

Application:
•Scholarship Application
•Scholarship Essay
•Two (2) letters of recommendations
•Proof of enrollment to the college/university being attended
•Final Transcript and Test Scores

Essay Prompt: (Typed, double spaced, 12-point Times New Roman, with numbered pages and one inch margin)
— In 500-700 words, write an essay in English or Spanish responding to the following topic: Describe how your heritage, family, and the community in which you grew up have impacted your desire and motivation to obtain a college degree. Additionally, be sure to include what you intend to accomplish with your degree and how you will give back to your community.
