Caring For Families Social Services is a support coordination agency that assists individuals in gaining access to needed NJ programs and NJ State plan services, as well as needed medical, social, educational and other services. Our vision is:

"To Empower Families in Need Live a Succesful Life." As a Support Coordination Agency we will assign a professional Support Coordinator, who will contact

the individual in need and or his/her family to initiate the person-centered planning process. The Support Coordinator is responsible for developing and maintaining the Individualized Service Plan (ISP) with the individual, the individual's family, and other team members designated by the individual. The Support Coordinator is also responsible for the ongoing monitoring of the provision of services included in the Individualized Service Plan.
