Tonight at 6:00 PM — “Care Planning Essentials: Preparing for Care at Home and Beyond.”
Free, virtual, one hour, with Registered Nurse Darlene Arroyo.
We’ll cover building a care plan: organizing essential information, preparing key documents, managing medications, coordinating among family and providers, and planning for transitions between care settings — at home, in assisted living, in a nursing facility, or across all of them over time.
Families who go in unprepared, without an informed advocate, often find the decisions are being made for them rather than with them. An hour tonight is how you change that.
