CMR reveals a heterogeneous regional myocardial response to long-term enzyme replacement therapy in Fabry disease. While glycosphingolipid storage reduction may improve tissue characteristics in some segments, other regions demonstrate persistent injury manifested by abnormalities on T2, ECV, and LGE.
