The past two years has been one of the most challenging periods ever for HQP. We were hit by a pandemic that viscously targeted our primary service population - vulnerable, older adults with complex needs and chronic conditions. Our nurses never wavered and never stopped seeing patients who needed us at home or in the nursing home - even at the start of the pandemic when most other health care providers drastically limited their service. Why? Because as a unique organization that not only provides care, but is dedicated to learning what really works to prevent avoidable complications, disability, and death ... we knew we were significantly contributing to a better health outcome during a time of great need. We follow the data.

There have been many other hardships HQP has endured during this time, including major losses of funding from organizational partners who suffered financially due to COVID. But, so far, with the help of old friends, new guardian angels and emerging partnerships we press on. Relentless in pursuing our aim to design, test, and spread advanced preventive care systems that help keep us all well ... no matter the challenges.

Our complex, chronically-ill, older adult patients and our specialized Advanced Preventive Care nurses have stayed strong all year long — through a pandemic, their devastating illnesses, and day after day of waiting and worrying.

Today on , a global day of giving back, we’re asking you to give just a little bit and help make the holidays special for the patients in our care. Consider sending a card to an elderly neighbor, a nurse friend, your doctor, or nursing home to lift the spirits of those around you.

According to the latest CDC data, 19.2% of COVID-19 cases in the US are in people 65+ but they account for 78.8% of all COVID-19 deaths.

In our system of Advanced Preventive Care, specially trained nurses work with this target population to create a sustained, supportive relationship that enables them to combine - care coordination, disease management, and personalized preventive interventions - to flexibly respond to changing patient needs over time including COVID-19. Our system has sometimes been described as "complex care management", "advanced care management", "community-based care management", and other labels, but we think the term Advanced Preventive Care best captures the broad, anticipatory, and proactive nature of our approach.
