Community health centers play a critical role in preventing and managing chronic disease, helping patients stay healthy and avoid more serious health complications.

Across the country in 2024 alone:
• 3.5 million patients received diabetes care — 72% reached A1c control
• 6.2 million patients received hypertension treatment — 67% achieved blood pressure control
• 10 million patients received obesity care — 72% of youth were screened and counseled on nutrition and physical activity

Together, community health centers serve 52 million people through the nation's largest primary care system, delivering innovative care where it matters most.
