Job Title: Nurse Practitioner (NP)
**Location:** SAFE Psychology and Wellness Center, Dearborn, MI
**Position Type:** Full-Time
**Reports To:** Medical Director/Primary Care Physician
**Job Summary:**
The SAFE Psychology and Wellness Center is seeking a dedicated and experienced Nurse Practitioner (NP) to join our multidisciplinary team. The NP will collaborate closely with the Primary Care Physician and other healthcare professionals to provide comprehensive primary care and wellness services. This role focuses on providing direct patient care, wellness counseling, preventive care, and health management in a community-focused mental health and wellness setting.
**Key Responsibilities:**
- Work collaboratively with the Primary Care Physician to assess, diagnose, and manage patient health concerns, including both physical and mental health needs.
- Perform routine exams, order and interpret diagnostic tests, and prescribe medication as needed.
- Provide wellness counseling, preventive care, and health education to promote healthy lifestyle choices.
- Offer support in managing chronic conditions and coordinate patient care across healthcare services.
- Conduct follow-up assessments to ensure effective treatment and management of conditions.
- Support the mental health services team in integrating wellness services with psychological care.
- Participate in developing and implementing health and wellness programs, including but not limited to nutrition, exercise, and mental health initiatives.
- Maintain accurate and up-to-date patient records and documentation.
- Ensure compliance with healthcare regulations and best practices.
- Collaborate with social workers, therapists, and other professionals to provide holistic approach in patient-centered care.
- Provide guidance to patients on referrals for specialized medical services as needed.
**Qualifications:**
- Valid Nurse Practitioner license in the state of Michigan.
- Master’s degree in Nursing or a related field from an accredited institution.
- Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) preferred.
- A minimum of 2 years of experience as a Nurse Practitioner, preferably in primary care, wellness, or mental health settings.
- Strong understanding of health promotion, disease prevention, and chronic condition management.
- Excellent communication and interpersonal skills, with the ability to work collaboratively in a multidisciplinary team.
- Experience with electronic health records (EHR) and patient management systems.
