Hello, Friends, Partners and Supporters,

CCPNH is closing effective 8/22/25 due to lack of federal funding. We have enjoyed working with each of you but unfortunately, we will no longer be available to address your needs.

The Care Coordination Program of NH thanks you for your support throughout the years.

We wish to be proactive about providing you with alternative resources.

Please reach out to your Family Programs Soldier & Family Readiness Specialists who are able to connect you with additional resources to include tax prep and financial counseling. They can be reached at the below intake number or follow the below link to submit a Wellness Division intake form.
