Wondering if you or a loved one may qualify for CCSP?

The Community Care Services Program (CCSP) helps seniors and individuals with physical disabilities live safely at home—or with family/caregivers—instead of moving to a nursing home.

Eligibility is based on the same medical, functional, and financial criteria used for Medicaid nursing home care. Our goal is to connect clients with resources that support staying in the community as long as possible.
