Facilitated by Certified Community Health Workers (CHWs) who are trusted members of the community with a unique perspective of the community’s needs, which enables them to develop culturally appropriate solutions to address infant mortality. Moms & Babies First is a free, voluntary service for eligible pregnant women that promotes black infant vitality by supporting healthy pregnancies and positive birth outcomes by reducing the health disparities related to infant mortality.

Products

Through ongoing visits to the home, or a location of the family’s choosing, Community Health Workers (CHWs) support parents in learning how to improve their family's health and provide better opportunities for their children. Home visits may include:

1. Comprehensive assessment and screenings;
2. Research-informed curriculum designed to reduce infant mortality by;
3. Supporting preventive health and prenatal practices (early access and use of OBGYN and primary care providers);
3. Initiation of healthy behaviors (nutrition, safe sleep, breastfeeding, immunizations, etc.);
4. Setting family goals, (reproductive life-planning, obtaining education, finding employment, etc.);
5. Referral and linkage to needed community-based medical and social supports; and
Facilitated transition to an appropriate development-enhancing setting.
