EAT MOVE SAVE Chicago An Illinois SNAP-ED program provided by the Chicago Partnership for Health Promotion

Founded in 2002, Chicago Partnership for Health Promotion (CPHP) is a unit of the Office of Community Engagement and Neighborhood Health Partnerships (OCEANHP) at the University of Illinois, Chicago representing a network of community-based interventions. CPHP is part of a SNAP-Ed Program, which uses evidence-based policy, systems, environmental and educational methods to improve nutrition and red

uce disparities in outcomes associated with nutritional diseases including obesity. It serves SNAP-eligible residents of Chicago and focuses its work in the 20 community areas of Chicago with the highest number of SNAP-eligible residents. As of 2016, CPHP is the largest community-based nutrition education program for low-income families in Cook County.
