Some of the biggest barriers to better health happen outside the exam room.

Housing instability, transportation challenges, financial strain, access to food, employment, mental health, and other circumstances can all play a role in a patient’s overall health and well-being.

CommuniCare helps participating providers address the bigger picture by giving them tools to identify these needs and connect patients with community resources and services across Wyoming.

It means that when something beyond medical care comes up, providers have a clearer way to help patients take the next step.

Heating a home can be a significant household expense, especially during Wyoming winters.

The Weatherization Assistance Program (WAP) helps eligible low-income households make their homes more energy efficient, which can save 5–25% on home heating bills.

Weatherization may include adding insulation, sealing leaks around doors and windows, and completing basic health and safety checks. These home improvements are provided at no cost to eligible applicants, and both renters and homeowners can qualify.

If you're looking for information about WAP or other utility assistance resources, dial 2-1-1 to get connected.
