Chesterfield County Distribution dates will be June 17th and June 18th at Hopewell Baptist Church Fellowship Building at15084 Hwy 9 in Chesterfield from 9am until 3pm each day. FIRST COME, FIRST SERVED

YOU MUST BE 60 YEARS OLD ON THE DAY OF APPLICATION.
YOU MUST KNOW TOTAL HOUSEHOLD SIZE AND INCOME
YOU MUST HAVE A PHOTO ID WITH CURRENT ADDRESS

CAN PICKUP FOR 2 OTHER PEOPLE -
- MUST HAVE A COPY OF THEIR ID
- MUST KNOW THEIR HOUSEHOLD SIZE AND INCOME
- MUST HAVE A NOTE NAMING YOU AS THE PERSON TO PICKUP VOUCHERS. NOTE MUST BE SIGNED BY INDIVIDUAL MATCHING THE ID.
- THIS INCLUDES SPOUSES.
- POA RECOGNIZED WITH PAPERWORK IN HAND.

YOU MUST APPLY IN THE COUNTY WHERE YOU LIVE.
