The Navigation Center supports individuals and families experiencing housing instability and crisis. Each has its own rules and procedures.

Through partnerships, integrated case management, and compassionate support, we help people move from crisis to stability—one relationship at a time. Military Community Connections (MCC) is a 501(c)3 nonprofit organization founded in 2015. MCC leaders have combined experience working with veterans and their families who are having difficulties transitioning into the community for over twenty-five years. In 2018 the city of Charleston asked us to open our doors to the general public. We did so and the Military Community Connections 501 (c)3 organization was renamed The Navigation Center. Military Community Connections is now a division of The Navigation Center. We help veterans, homeless people, families and mentally ill and addicted people navigate a very complex ecosystem of Gov’t and NFP organizations. The Navigation Center conducts a complete intake and assessment to identify all factors leading to or impacting our clients. TNC is able to work with each client to develop an appropriate plan to address not only the presenting need, but to ensure that they are able to eliminate the underlying causes. There are more than 30 + of organizations in our community that may provide services needed by our client. As you might expect these rules and procedures can be very confusing and frustrating particularly for those in crisis. Additionally, there are also many minor and sometimes major gaps in services needed to help those in need to reestablish their independence. This means people need a guide, advocate, coach and a cheerleader to successfully navigate this maze. This is what the people at The Navigation Center provide.

FOUR WAYS TO HELP

01 FUND A FAMILY — $500 walks one family from crisis to stability: rent or utilities covered, prescriptions filled, and a caseworker who stays.

02 AMAZON WISHLIST — an easy way to provide items for TNC's blessing bags for kids and adults, as well as gift cards for direct family support.

03 VOLUNTEER — complete the form to volunteer for nightly dinners, blessing bag assembly, and office help.

04 FOLLOW & SHARE — follow us, then share this post. One share reaches someone who can help.

THE HEALING ECOSYSTEM

Both Navigator Programs are supported by TNC's Healing Ecosystem — the coordinated network of community navigator partnerships that addresses the full spectrum of a client's needs.

Across three pillars of care — Health, Housing, and Human Services — TNC coordinates every piece of the journey until stability and hope are present.

This is a person-centered, trauma-informed model that addresses root causes, not just symptoms.

TNC's work is organized around two primary pathways — one for families, one for individuals. Different pathways, one promise: we walk alongside.

FAMILY NAVIGATOR PROGRAM addresses the parent's emergency and the children's stability at the same time. Serving families referred through CCSD schools, walk-in families in immediate crisis, single mothers navigating housing instability, and families experiencing the 'hidden homeless' situation of long-term hotel stays.

INDIVIDUAL NAVIGATOR PROGRAM walks alongside adults navigating crisis alone. Serving veterans, individuals discharged from hospitals or treatment facilities, those referred through street outreach, individuals exiting incarceration, and adults facing eviction, chronic illness, or behavioral health needs.

FROM INTAKE TO IMPACT: THE SIX-STAGE JOURNEY

Whether a neighbor walks through TNC's door on their own or is referred by a school, hospital, or partner organization, the journey is the same.

01 ENTRY POINT — walk-in or referral

02 TRIAGE INTAKE — 2 to 6 hours

03 CRISIS STABILIZATION — 8 to 20 hours

04 COMPREHENSIVE CASE MANAGEMENT — ongoing

05 HOUSING STABILITY

06 STABILITY & ONGOING SUPPORT

The journey doesn't end at stability. Every client can always return, reconnect, and continue moving forward.

"TNC is set apart because we coordinate and track the journey. We literally walk alongside them. We will make sure they have their prescriptions, schedule an Uber to their doctor's appointment, and continue to follow up to assure long-term stability and hope."

— Marie Elena Roland, Founder and Executive Director

HOW TNC IS DIFFERENT

Charleston has many organizations doing important work in housing, healthcare, and crisis support. TNC's distinction is not just what we do, it is how we do it.

1. WE ACCOMPANY. Other organizations refer. TNC accompanies. We don't hand someone a list of resources and wish them well — we walk the path with them, every step.

2. WE COORDINATE. Other organizations operate in silos. TNC aligns housing, healthcare, mental health, employment, and human services into one unified path forward.

3. WE ARE NOT A SHELTER. We deliver shelter-level comprehensive services to people who cannot or will not enter a shelter, filling a critical, underserved gap.

THE NUMBERS BEHIND THE NEED

Every statistic represents not a data point, but a family that stayed housed, a child who stayed in school, and a person who kept their lights on long enough to find stability.

Numbers tell scale. Stories tell meaning. We use both.

Thanks to our partnership with the Charleston Atlantic Presbytery The Navigation Center now has a second location in North Charleston:

📍 4701 Park Pl W, North Charleston, SC 29405.
Open Thursdays | 9:00 AM–3:00 PM

This location puts us closer to the families we walk alongside, cutting the transportation barriers that too often stand between a neighbor and the help they need.

Through our partnership with MUSC CARES Clinic for the Unhoused, telehealth hours will also be available on site to address immediate healthcare needs.

Our downtown location remains open Monday–Friday, 9:00 AM–5:00 PM, with our full continuum of services.

We are grateful to the Charleston Atlantic Presbytery for standing with our mission and helping us meet people where they are.

Two locations. Fewer barriers. One mission, walking alongside our most vulnerable neighbors toward stability and hope.

WHO WE ARE

Founded in 2015 and based in Charleston, South Carolina, The Navigation Center is a systems-level intermediary rooted in a simple belief: no one should have to navigate a crisis alone.

TNC does not operate as a single-service provider. We function as a central coordinating hub, ensuring individuals and families experiencing housing instability can navigate and access the full ecosystem of support required to achieve long-term, independent, sustainable living.
