There has been some confusion regarding whether TRICARE covers doula services for those delivering at a Military Treatment Facility (MTF). At this time, the answer is no—doula support is not covered when the birth takes place at an MTF under the Childbirth and Breastfeeding Support Demonstration (CBSD).

If you’d like to learn more about the CBSD, you can review the official details through TRICARE resources.

It’s also important to understand that, under the CBSD:

Doulas must be TRICARE-authorized and participating providers
You cannot use an out-of-network doula and submit a superbill for reimbursement
The doula is responsible for filing claims directly with TRICARE
Doulas may not charge upfront for covered services under this program

Understanding these guidelines can help families avoid unexpected costs and ensure they are working with eligible providers.

The TRICARE Childbirth and Breastfeeding Support Demonstration covers support services from certified non-medical labor doulas, certified lactation consultants, and certified lactation counselors. You can use the CBSD if you have TRICARE Prime, TRICARE Prime Remote, or TRICARE Select.

🌸 It’s been a little while since we’ve introduced a new member, and we are so excited to change that today! 🌸

Please join us in welcoming Lauren Dornell Neal with DOULA SOLL, supporting families in the Camp Lejeune Military Birth Resource Network & Postpartum Coalition area.

Lauren is a DONA-trained Birth Doula as well as a Licensed Clinical Mental Health Counselor (LCMHC) and Licensed Clinical Addictions Specialist (LCAS) in NC. She looks forward to offering compassionate and adaptable support for families on their birthing journeys. 💜

The event will include vendors, lots of giveaways, a variety of resources for families, information on breastfeeding, lactation consultants, carseat checks, and other support services. TRICARE CBSD FAQ – Clearing Up Common Questions

We often receive questions—and hear quite a bit of misinformation—about how the TRICARE Childbirth and Breastfeeding Support Demonstration (CBSD) benefit works. I wanted to take a moment to share a few frequently asked questions and important clarifications to help both beneficiaries and providers navigate this benefit correctly.

1. WHERE CAN A BENEFICIARY BE SEEN?

The CBSD benefit only applies to care received off base by a TRICARE-approved provider. Unfortunately, this means services provided through the Military Treatment Facility (MTF) are not covered under this benefit. Additionally, if a beneficiary chooses a homebirth midwife who is not TRICARE-authorized, the CBSD benefit does not apply.

2. PRIME BENEFICIARIES USING OUT-OF-NETWORK PROVIDERS

If you are a TRICARE Prime beneficiary and wish to see an out-of-network doula, you may need a referral from your Primary Care Manager (PCM). Without a proper referral, the benefit could be denied, and you may incur out-of-pocket costs.

3. SUPERBILLS AND DOULA BILLING

As of January 1, 2025, all doulas must be participating TRICARE providers and are required to bill TRICARE directly for services, whether they are in-network or out-of-network. Providing superbills for beneficiary reimbursement is no longer allowed.

4. ADDITIONAL SERVICES AND BILLING PRACTICES

Doulas may offer non-covered services outside the CBSD benefit (such as placenta encapsulation or postpartum support), but these must be clearly listed as separate and optional. Importantly:

• Doulas cannot require a deposit for covered services under the CBSD benefit.

• Balanced billing is prohibited. For example, if a doula charges $2,000 and TRICARE reimburses $1,000, the doula cannot charge the beneficiary the remaining $1,000.

5. CERTIFICATION REQUIREMENTS

The CBSD benefit only covers labor doulas, not postpartum doulas. A doula must hold certification as a Labor Doula from a TRICARE-approved organization. While a doula may be certified as a postpartum doula, that credential alone does not qualify under the CBSD policy.
Note: As of now, North Carolina Medicaid does not recognize doula services as a covered benefit, regardless of certification.
