Trent's Team Transitional Services Transitional program for young adults ages 18-21 with developmental disabilities transitioning out of high school.

We help to develop independent living skills through education, experience, modifications, volunteer opportunities and job coaching.

Trent's Team Transitional Services has now opened in Calhan, CO. We provide services to young adults 18-21 with Cognitive, Developmental and/or Physical Disabilities. We focus on helping individuals achieve their goals and develop life skills through mentoring, volunteering, hands on training, job coaching, modifications and adaptations.
