Now Hiring: Moving Home Minnesota (MHM) Specialist
📍 Locations: Wabasso, Olivia, and Franklin, Minnesota
Are you passionate about helping individuals achieve independence and successfully transition into community living? We are seeking compassionate, organized, and motivated professionals to join our team as a Moving Home Minnesota (MHM) Specialist.
Position Summary
The Moving Home Minnesota Specialist supports individuals transitioning from institutional or provider-controlled settings into homes of their own within the community. This role focuses on person-centered planning, community integration, housing stabilization, resource coordination, and ongoing support to ensure successful independent living outcomes.
Responsibilities
Assist individuals with the transition from institutional or provider settings into community-based housing
Coordinate housing resources, community services, and support systems
Develop and implement person-centered transition plans
Advocate for individuals' rights, choices, and goals
Connect individuals with community resources, healthcare providers, and social services
Monitor progress and provide ongoing support during the transition process
Maintain accurate documentation and comply with DHS and agency requirements
Collaborate with families, guardians, case managers, and community partners
Qualifications
Experience working with individuals with disabilities, mental health needs, or vulnerable populations preferred
Strong communication, organization, and problem-solving skills
Ability to work independently and manage multiple responsibilities
Valid driver's license and reliable transportation
Ability to pass required background studies
Knowledge of Minnesota DHS programs and services is a plus
