I’ve watched the videos of the Damar Hamlin collapse last night and first and foremost, my prayers are with him. I’ve seen lots of post wondering what could have happened to cause cardiac arrest in a seemingly very healthy young athlete. I’m not going to try to make a diagnosis, but state what I feel is the most likely scenario.

Obviously all things must be tested and ruled out to make sure there are no other underlying risk, but the most likely scenario is a phenomenon known as “commotio cordis”. This happens when a young athlete is struck in the left chest at precisely the right millisecond as the ventricles(the bottom chambers of the heart), are repolarizing. As you see in the picture, this is a very short window in the cardiac cycle. The sudden blunt force applied to the chest, causes the ventricles to instead depolarize and quiver. This is called Ventricular fibrillation. This causes the blood pressure to drop to nothing and this leads to cardiac arrest. It usually takes a few seconds for the blood pressure to drop, thus is why Hamlin was able to stand up before immediately falling back to the ground. The immediate best treatment for this is defibrillation with an Automated external defibrillator. Early defibrillation gives a great chance at a great outcome. Again, there may be other underlying factors, but this is the most likely scenario.

What is my purpose with this post? To show the importance of having AEDs at all sports functions. At any sport, no matter what it is, there needs to be an AED. it must be in plain sight so people know where it is. If you are running a sports program and you do not have one, get one. They are fairly inexpensive and save lives. If you want more information about AEDs, let me know or see any local EMS agency. They can give you all the facts and ways to obtain an AED.
