Recognizing its impact is an important step toward change. At Community Health Center of Buffalo, Inc., our team of counselors and nurse practitioners are here to help you better understand what you’re experiencing and to build the skills needed to manage anxiety so you can live a more fulfilling life.

Huge shoutout to our Screening & Navigation Team Lead Desiree, who was on-site making real connections and helping community members take the next step toward stability. This is the work that matters — sitting down one-on-one, breaking things down, and getting people actually linked to services through the Medicaid 1115 Waiver.

This isn’t just information — this is access.

Through the 1115 Waiver, individuals can get connected to:

* Housing supports & transition services
* Pre-tenancy & tenancy sustaining services
* Nutrition assistance
* Case management
* Transportation for health-related social needs

Know that if you need to be trained or your staff and or colleagues. Family or friends feel they could and WOULD (they will), benefit greatly from knowing CPR and Dfib- contact us directly @ WNY mobile ops and we can make that happen for you; with our NYS certified CPR instructors teaching you how it’s never been easier saving lives. The more you know the better equipped we are as passersby, community members and family or friends to help save a life or keep one going until emergency and first responders are available and on scene to take over.

Community as one , knowledge is power; advocate and educate.
