The CVHHH team has been busy implementing a new electronic health record, or EHR, with our partner, Homecare Homebase. A leader in it's field, Homecare Homebase works with home health and hospice providers nationwide.

An EHR is a digital platform that stores essential patient and organizational information in one secure, integrated system. It's where clinicians document patient information during visits and can communicate about a person's progress meeting goals, it's where patient information gets packaged to be shared with local provider partners, and it's where our quality team gains insights into opportunities to improve how we care for Central Vermonters at home.

Launching a new EHR is a core pillar of CVHHH's strategic plan. We are confident that when we go live with Homecare Homebase in September, we will be in better shape than ever to meet the needs of our patients, healthcare partners, and employees.

This week, Homecare Homebase is onsite at our headquarters in Barre to teach some members of Team CVHHH how to use the new system. Look how excited everyone is to dig in, click around, and learn! Thanks to our users, pictured here, for pioneering this technology and leading this transition for the organization!
