The Willow Foundation is focused on people dedicated to helping people. Our mission is to
serve and protect the values of the community. We believe that every individual, regardless of
location or financial means, deserves to have access to the full spectrum of home-based
healthcare, community resources, and education. The Willow Foundation was founded with the
vision that we need to come together
as a community to help those in need of care, assistance,
resources and information that support local communities in providing financial assistance to
non-funded patients for healthcare costs, as well as fulfillment of a patientβs last wishes, and to
ensure the basic needs for comfort are met. The Willow Foundation wants to help our
community to thrive with an abundance of resources and education. We are here to support our
partners in their efforts to uplift our community aiding with fundraising, drives, advertising, and
other such supportive programs. We are here to help individuals with funeral costs, last wish
funds, and emergency help. Too often people are left with no means and simply have not been
provided with the education to help them succeed. There are many programs that people miss out
on due to lack of information. The Willow Foundation will provide community outreach and
education to bridge the knowledge gap and make these programs accessible to the community. The Willow Foundation will fundraise, apply for grants, hold drives, and accept donations to
provide financial support for:
- The development and expansion of
Healthcare Services to provide:
o Physicianβs services in rural and underserved communities
o Home Healthcare in rural and underserved communities
o Home based senior care services in rural and underserved communities
o Home Hospice in rural and underserved communities
o Home based Palliative Care in rural and underserved communities
- Provide social and financial assistance to non-funded and under-privileged patients for:
o Healthcare access
o Medications
o Fulfill last wishes
o Funeral services
o Basic comfort needs
- Provide education to communities that include, but are not limited to:
o Physicianβs office
o Hospitals
o Community Centers
o Assisted living facilities
o Long-term care Facilities
o Patients and their families
