At the Summit County Continuum of Care, we lead a county-wide commitment to ending homelessness. Guided by a faith-driven purpose, we bring together nonprofit providers, local and state partners, and community stakeholders to drive solutions that quickly rehouse individuals and families while minimizing disruption. Together, we are building a coordinated system that prevents homelessness, responds swiftly when it occurs, and moves people into safe, stable housing.

We’re proud to announce that the Summit County Continuum of Care has received a $2,500 grant to support our Mobility Is Stability Initiative. This funding will help us provide transportation assistance like transit passes, gas cards, and ID support to individuals and families facing housing instability—helping remove barriers to employment, housing, and essential services in our community.

The Lived Experience Committee ensures that individuals who are currently or formerly homeless play a meaningful role in shaping SCCoC projects. Members engage across areas such as research, consultation, program support, communications, policy, and advocacy, while also providing trauma-informed, strength-based support in roles like outreach, case management, recovery, and peer support!

Through HUD/SCCoC-funded programs, CSS ensures that people experiencing the highest barriers still have access to compassionate, consistent support. Donor contributions strengthen these outreach efforts by helping supply necessities, transportation assistance, and field materials.
When outreach teams have the tools they need, more lives can be reached — and changed. Learn more and give today by visiting summitcoc.org/financial-support
