凡設籍或實際居住生活於桃園市，年滿6歲至未滿65歲，領有身心障礙證明（手冊）者。
＊服務內容：
1.身心障礙者個案管理及轉銜服務。
2.整合在地化身心障礙社會福利資源。
3.規劃辦理身心障礙者家庭支持服務方案等並協助重建生活。
