🇹🇼 【T-RUST】台灣 IBD 真實世界研究
台灣腸道疾病研究醫學會(TASID)主導的 T-RUST 台灣多中心真實世界研究,已正式發表於發炎性腸道疾病領域的指標性國際期刊 《Inflammatory Bowel Diseases (IBD Journal)》!
這項研究透過 TASID IBD Registry,串聯全台 15 家主要醫學中心與大型醫院,共同累積屬於台灣本土 IBD 病人的真實世界治療證據。
📌 貼近真實轉診生態的複雜收案
研究共納入 379 位接受喜達諾Ustekinumab (Stelara) 治療的發炎性腸道疾病病人(301 位克隆氏症、78 位潰瘍性結腸炎)。
這並不是一群「單純、容易治療」的病人。超過 7 成病人過去曾接受過其他進階治療,且整體約有 1/3 屬於「難治型發炎性腸道疾病(Difficult-to-Treat IBD)」。這相當貼近台灣大型 IBD 轉診中心實際面對的複雜臨床情境。
📌 T-RUST 研究帶來四大重要訊息:
✅ 一年治療持續率高
滿一年時,克隆氏症治療持續率達 91.0%,潰瘍性結腸炎達 85.9%。即使病人過去曾接受生物製劑、多線進階治療,甚至屬於難治型 IBD,整體治療持續性依然維持良好。
✅ 52 週仍維持良好的臨床效果
在有完整一年追蹤資料的病人中,臨床緩解率分別達到克隆氏症 84.7%、潰瘍性結腸炎 90.2%。在「不使用類固醇」的狀態下仍維持臨床緩解的比例,也分別高達克隆氏症 76.1%、潰瘍性結腸炎 82.9%。
✅ 瘻管型克隆氏症也看到重要成果
在可評估的活動性瘻管型克隆氏症病人中,約 45.9% 達到瘻管完全閉合(其中肛門周圍瘻管閉合率為 45.5%)。值得注意的是,這些達到瘻管閉合的病人,皆是在研究期間未合併手術介入的情況下,純由藥物治療達成。
✅ 整體安全性良好
研究期間沒有發現新的安全性警訊,也沒有任何病人因藥物不良反應而停止 Ustekinumab 治療。
🌏 為什麼台灣自己的真實世界研究很重要?
亞洲 IBD 病人的疾病特徵、治療順序、醫療制度及藥物可近性,都與歐美不完全相同。
因此,除了國際大型臨床試驗,我們更需要來自台灣臨床現場的真實資料,才能精確了解治療在不同族群、不同治療線別,以及難治型 IBD 中真正的長期表現。
這正是 TASID 致力建立 全台 IBD Registry 與多中心研究平台的重要目的:
把每一家醫院累積的臨床經驗串聯起來,將台灣的真實世界資料,轉化成具備國際影響力的醫學證據!

🇹🇼 [Academic News] Taiwan's Real-World IBD Study Shines on the International Stage!
We are thrilled to share that the T-RUST study (Taiwan Multicenter Real-world Ustekinumab Study), led by the Taiwan Association for the Study of Intestinal Diseases (TASID), has been officially published in the international journal, Inflammatory Bowel Diseases (IBD Journal)!
This study utilized data from the TASID IBD Registry, connecting major medical centers and hospitals across Taiwan to gather real-world treatment evidence specifically for Taiwanese IBD patients.
📌 A Complex Cohort Reflecting Real-World Clinical Practice
The study included a total of 379 patients with inflammatory bowel disease treated with ustekinumab, comprising 301 patients with Crohn's disease and 78 with ulcerative colitis.
This was not a simple, easy-to-treat cohort:
Over 70% of patients had prior exposure to advanced therapies. Approximately one-third of the cohort met the criteria for difficult-to-treat (DTT) IBD. This closely mirrors the highly complex clinical scenarios faced by major IBD referral centers in Taiwan.
📌 Four Key Takeaways from the T-RUST Study:
✅ High 1-Year Treatment Persistence
At the one-year mark, ustekinumab persistence reached 91.0% for Crohn's disease and 85.9% for ulcerative colitis. Drug persistence remained favorable regardless of prior biologic exposure, varying histories of advanced therapy exposure, or DTT IBD status.
✅ Sustained Clinical Effectiveness at 52 Weeks
Among patients with available week 52 data, clinical remission was achieved in 84.7% of Crohn's disease patients and 90.2% of ulcerative colitis patients. Steroid-free clinical remission rates reached 76.1% for Crohn's disease and 82.9% for ulcerative colitis.
✅ Significant Achievements in Fistulizing Crohn's Disease
In evaluable patients with active fistulizing Crohn's disease, 45.9% achieved fistula closure. The perianal fistula closure rate was 45.5%. Notably, fistula closure was achieved entirely through medical therapy, with no patients requiring concomitant surgery throughout the study.
✅ Favorable Overall Safety Profile
Ustekinumab was generally well tolerated, with no new safety signals identified. Importantly, no adverse event led to the discontinuation of ustekinumab in either cohort.
🌏 Why is Taiwan's Own Real-World Research So Important?
Taiwanese patients may exhibit distinct clinical characteristics, treatment histories, and genetic backgrounds, which underscores the need for local real-world data. Beyond global clinical trials, real-world evidence from our own clinical frontline is essential to optimize patient care.
This is the core mission behind TASID's dedication to establishing the TASID IBD Registry and our multicenter research platform.
