𝗣𝗲𝘁𝗲𝗿 𝘂𝗽𝗱𝗮𝘁𝗲 ❤️🩹
Last week we met with Peter’s oncologist to discuss his biopsy results and the treatment options available to him. Unfortunately, the results confirmed malignant melanoma, an aggressive cancer in his oral cavity with a high rate of spreading, particularly to the lymph nodes and lungs.
Peter’s cancer has grown into his maxilla (upper jaw bone), where it cannot be completely removed with surgery. His CT scan also showed enlarged lymph nodes, so there is concern that the cancer may already have spread to them.
Surgery is possible to remove the large tumour in his mouth, along with surrounding gum tissue and part of his upper jaw. It would be a very aggressive procedure, intended to prevent further bleeding and remove necrotic tissue that could lead to serious infection and sepsis. Following surgery, Peter would need an oesophagostomy feeding tube for around 7–10 days while he recovered.
He would then need chemotherapy to help reduce the cancer cells and slow the progression of the disease. Sadly, this treatment would be palliative, not curative. The cancer cannot be completely removed and will continue to progress.
This leaves us with an incredibly difficult decision. We don't want to put our old boy through major surgery and a difficult recovery simply to buy him more time if that time will not give him a good quality of life.
Peter is still eating with our help. We syringe-feed him and give his medications every day. Sadly, he is now lethargic every day and cries when we need to move him, which makes us concerned that he is experiencing pain. 💔
Tomorrow Peter will return to the hospital to be reassessed by his oncologist and surgeon. We need to understand how he is doing now before making a decision about what is best for him.
Sadly, Peter’s prognosis is poor. At this stage, our priority is not simply how much more time we can give him, but the quality of that time.
Whatever happens next, Peter’s comfort and wellbeing will come first. ❤️
อัปเดตอาการของปีเตอร์ ❤️🩹
เมื่อสัปดาห์ที่แล้ว เราได้พบกับคุณหมอด้านมะเร็งของปีเตอร์เพื่อพูดคุยเกี่ยวกับผลตรวจชิ้นเนื้อและทางเลือกในการรักษาที่มีอยู่ แต่น่าเสียดายที่ผลตรวจยืนยันว่า ปีเตอร์เป็น มะเร็งเมลาโนมาชนิดร้าย (Malignant Melanoma) ซึ่งเป็นมะเร็งที่มีความรุนแรงในช่องปากและมีอัตราการแพร่กระจายสูง โดยเฉพาะไปยังต่อมน้ำเหลืองและปอด
มะเร็งของปีเตอร์ได้ลุกลามเข้าไปใน กระดูกขากรรไกรบน (maxilla) ซึ่งไม่สามารถผ่าตัดนำมะเร็งออกได้ทั้งหมด ผล CT scan ยังพบว่าต่อมน้ำเหลืองมีขนาดโตขึ้น จึงมีความกังวลว่ามะเร็งอาจแพร่กระจายไปยังต่อมน้ำเหลืองแล้ว
สามารถผ่าตัดนำก้อนมะเร็งขนาดใหญ่ในช่องปากออกได้ รวมถึงเนื้อเยื่อเหงือกโดยรอบและกระดูกขากรรไกรบนบางส่วน แต่จะเป็นการผ่าตัดใหญ่และรุนแรง จุดประสงค์คือเพื่อป้องกันไม่ให้ก้อนมะเร็งมีเลือดออกอีก และนำเนื้อเยื่อที่ตายออกเพื่อป้องกันการติดเชื้อรุนแรงและภาวะติดเชื้อในกระแสเลือด (sepsis) หลังการผ่าตัด ปีเตอร์จะต้องใส่สายให้อาหารผ่านหลอดอาหาร (esophagostomy tube) ประมาณ 7–10 วันในช่วงพักฟื้น
หลังจากนั้นเขาจะต้องได้รับเคมีบำบัด เพื่อช่วยลดเซลล์มะเร็งและชะลอการลุกลามของโรค แต่น่าเศร้าที่การรักษานี้เป็นเพียง การรักษาแบบประคับประคอง (palliative care) ไม่ใช่การรักษาให้หายขาด มะเร็งไม่สามารถนำออกได้ทั้งหมดและจะยังคงลุกลามต่อไป
นี่จึงเป็นการตัดสินใจที่ยากมากสำหรับเรา เราไม่อยากให้ปีเตอร์ซึ่งเป็นสุนัขสูงวัยของเราต้องผ่านการผ่าตัดใหญ่และการพักฟื้นที่ยากลำบาก เพียงเพื่อยืดเวลาออกไป หากเวลาที่เพิ่มขึ้นนั้นไม่ได้มาพร้อมกับคุณภาพชีวิตที่ดี
ปีเตอร์ยังคงกินอาหารได้ด้วยความช่วยเหลือจากเรา เราป้อนอาหารด้วยไซริงค์และให้ยาเขาทุกวัน แต่น่าเศร้าที่ตอนนี้เขาซึมและอ่อนแรงทุกวัน และร้องเมื่อเราต้องขยับตัวเขา ทำให้เรากังวลว่าเขากำลังเจ็บปวด 💔
พรุ่งนี้ปีเตอร์จะกลับไปโรงพยาบาลอีกครั้ง เพื่อให้คุณหมอด้านมะเร็งและศัลยแพทย์ประเมินอาการของเขาอีกครั้ง เราจำเป็นต้องรู้ว่าอาการของเขาในตอนนี้เป็นอย่างไรก่อนที่จะตัดสินใจว่าอะไรคือสิ่งที่ดีที่สุดสำหรับเขา
น่าเศร้าที่การพยากรณ์โรคของปีเตอร์ไม่ดี ในตอนนี้ สิ่งสำคัญสำหรับเราไม่ใช่เพียงว่าเราจะสามารถยืดเวลาให้เขาได้อีกนานแค่ไหน แต่คือ คุณภาพของเวลาที่เขายังมีอยู่
ไม่ว่าเราจะต้องตัดสินใจอย่างไรต่อจากนี้ ความสบายและคุณภาพชีวิตของปีเตอร์จะต้องมาก่อนเสมอ ❤️
