การแยกงบ HIV/เอดส์ออกจากระบบเหมาจ่ายรายหัว เป็นหนึ่งในกลไกสำคัญที่ทำให้ผู้ติดเชื้อรายใหม่ของไทยลดลงจากหลักแสนเหลือ 8,000-10,000 คนต่อปี เพราะเปิดทางให้ทั้งภาครัฐ เอกชน และภาคประชาสังคมเข้ามาร่วมดูแลได้มากขึ้น

จากประสบการณ์ทำงานร่วมกับภาคประชาสังคม IHRI เห็นตรงกันว่า บริการที่นำโดยชุมชนจะยั่งยืนได้ ต้องมีนโยบายและงบประมาณภายในประเทศรองรับอย่างมั่นคง

ชี้แยกงบ “HIV/เอดส์” จากเหมาจ่ายรายหัว ช่วยคนเข้าถึงบริการ เสนอเพิ่ม 300 ล้าน ลุยค้นหาเชิงรุก

IHRI ยินดีเป็นอย่างยิ่งที่ได้ร่วมเป็นส่วนหนึ่งของงาน Pacific HIV Symposium 2026 โดยคุณรีน่า จันทร์อำนวยสุข ผู้อำนวยการฝ่ายวิจัยเชิงปฏิบัติการและนวัตกรรม ได้ขึ้นแบ่งปันประสบการณ์ของประเทศไทยในหัวข้อ “Community-Led Innovation in HIV Testing: Lessons Learned from Thailand”

เนื้อหาที่นำเสนอแสดงให้เห็นว่า บริการสุขภาพที่นำโดยชุมชนมีบทบาทสำคัญเพียงใดในการขับเคลื่อนงานตรวจและป้องกันเอชไอวีของไทย ไม่ว่าจะเป็นบริการเพร็พ การตรวจเอชไอวีด้วยตนเอง ไปจนถึงการบูรณาการบริการแบบยึดผู้รับบริการเป็นศูนย์กลาง ที่ครอบคลุมทั้งสุขภาพทางเพศ บริการสุขภาพที่ยืนยันอัตลักษณ์ทางเพศ สุขภาพจิต และการลดอันตรายจากการใช้สารเสพติด

สิ่งหนึ่งที่ถูกเน้นย้ำในงานนี้คือ การนำโดยชุมชน นวัตกรรม นโยบายระดับประเทศที่เอื้อต่อการทำงาน และงบประมาณภายในประเทศที่ยั่งยืน คือปัจจัยสำคัญที่จะทำให้บริการที่นำโดยชุมชนกลายเป็นส่วนหนึ่งของระบบสุขภาพ และเดินหน้าต่อไปได้อย่างยั่งยืนในระยะยาว

IHRI was glad to take part in the Pacific HIV Symposium 2026, where Ms. Rena Janamnuaysook, Director of Implementation Research and Innovation, spoke about Thailand’s experience in “Community-Led Innovation in HIV Testing: Lessons Learned from Thailand.”

Her presentation looked at how community-led health services have reshaped HIV testing and prevention in Thailand — from differentiated PrEP and HIV self-testing to integrated, person-centred care spanning sexual health, gender-affirming care, mental health, and harm reduction.

One message stood out: community leadership, innovation, supportive national policy, and sustainable domestic financing are what allow community-led services to become part of the health system and last for the long run.

Our thanks to the Fiji Ministry of Health and Medical Services, ASHM, and Roche for the chance to exchange experiences and learn alongside colleagues from across the Pacific.

On 14 August 2026, Dr. Jakkrapatara Boonruang, Medical Team Lead, represented the Institute of HIV Research and Innovation (IHRI) as a panelist at the inaugural Inclusion Champion Awards Ceremony & Panel Discussion, “Honoring Excellence in Inclusive Health and Shaping the Path Forward,” in Vientiane, Lao PDR.

Dr. Boonruang shared the key framework adopted by IHRI to ensure that HIV services are responsive to community needs: demedicalization, simplification, and differentiation. These principles have been instrumental in advancing key population-led PrEP and other HIV services in Thailand—bringing care closer to communities and making it more accessible, acceptable, and inclusive.

11 สิงหาคม 2569 — แพทย์หญิงสิรินทร์ ตั้งเจริญเสถียร จากสถาบันเพื่อการวิจัยและนวัตกรรมด้านเอชไอวี (IHRI) ได้เข้าร่วม การประชุมเชิงปฏิบัติการระดับภูมิภาคว่าด้วยการเรียนรู้ร่วมกันด้านการจ่าย Lenacapavir (LEN) PrEP ซึ่งจัดขึ้นระหว่างวันที่ 11–13 สิงหาคม 2569 ณ กรุงมะนิลา ประเทศฟิลิปปินส์

ใน Session 3: Lessons Learned from PURPOSE 2 แพทย์หญิงสิรินทร์ ได้นำเสนอองค์ประกอบสำคัญที่จำเป็นต่อการเตรียมความพร้อมสำหรับการนำ LEN ไปใช้ โดยถ่ายทอดประสบการณ์ของ IHRI ในฐานะหนึ่งในหน่วยวิจัยของโครงการ PURPOSE 2 การนำเสนอเน้นถึงความสำคัญของการให้ความรู้แก่ชุมชนและการสร้างความต้องการบริการ การพัฒนาศักยภาพผู้ให้บริการ การจัดทำแนวทางปฏิบัติงานมาตรฐาน (Standard Operating Procedures) ที่ชัดเจน การตรวจเอชไอวีที่มีผลรวดเร็ว การให้คำปรึกษาและการดูแลอาการไม่พึงประสงค์บริเวณจุดฉีด ตลอดจนการวิจัยด้านการนำไปปฏิบัติ เพื่อสนับสนุนการนำ LEN ไปใช้ในบริการ PrEP ภายใต้บริบทการให้บริการจริง

ประเด็นสำคัญประการหนึ่งคือ ความสำเร็จของการขยายบริการ LEN ไม่ได้ขึ้นอยู่เพียงแค่การฉีดยาเท่านั้น แต่ยังต้องอาศัยระบบบริการสุขภาพและผู้ให้บริการที่สามารถเตรียมความพร้อม ให้คำปรึกษา และสนับสนุนผู้รับบริการตลอดเส้นทางการป้องกันเอชไอวี โดยต้องทำให้บริการเข้าถึงได้ง่ายและยึดผู้รับบริการเป็นศูนย์กลาง แพทย์หญิงสิรินทร์ ยังเน้นย้ำว่า แม้บทเรียนจากการวิจัยทางคลินิกจะเป็นพื้นฐานสำคัญ แต่ การวิจัยด้านการนำไปปฏิบัติภายในแต่ละประเทศมีความจำเป็นอย่างยิ่ง เพื่อทำความเข้าใจอุปสรรคในบริบทท้องถิ่น ประเมินความเป็นไปได้และการยอมรับของบริการ และค้นหาแนวทางที่จะช่วยให้การดำเนินงานมีความยั่งยืนในระยะยาว รวมถึงการสนับสนุนงบประมาณจากแหล่งทุนภายในประเทศ

11 Aug 2026 — Dr. Sirin Tangcharoensathien from the Institute of HIV Research and Innovation (IHRI) participated in the Lenacapavir (LEN) PrEP Regional Peer Learning Workshop, held from 11–13 August 2026 in Manila, the Philippines.

During Session 3: Lessons Learned from PURPOSE 2, Dr. Sirin presented on the core components needed to prepare for LEN implementation, drawing on IHRI’s experience as a PURPOSE 2 study site. Her presentation highlighted the importance of community education and demand generation, provider training, clear Standard Operating Procedures, timely HIV testing, counselling and management of injection site reactions, and implementation research to support the introduction of LEN into real-world PrEP services.

A key takeaway was that successful LEN rollout goes beyond providing an injection. It requires health systems and providers to prepare, counsel, and support clients throughout their prevention journey, while ensuring that services remain accessible and person-centered. Dr. Sirin also emphasized that, while lessons from clinical trials provide an important foundation, in-country implementation research is essential to better understand local barriers, assess feasibility and acceptability, and identify approaches that can support long-term sustainability, including through domestic financing.

IHRI gratefully acknowledges the support of the U.S. Department of State and the EpiC project implemented by FHI 360 for supporting this regional peer learning opportunity and strengthening collaboration among countries preparing for LEN rollout.

10 Aug 2026 — IHRI Visits CU Innovation Hub (CUiHUB) at Chamchuri Square, Bangkok

Dr. Nittaya Phanuphak, Rena Janamnuaysook, Dr. Jakkrapatara Boonruang, and Nut Pullasiri from the Institute of HIV Research and Innovation (IHRI) visited the CU Innovation Hub (CUiHUB) at Chamchuri Square, Bangkok, to congratulate the team on the successful establishment of the Hub.

CUiHUB is Chulalongkorn University’s platform for supporting innovation and entrepreneurship, bringing together researchers, entrepreneurs, industry partners, and communities to turn ideas and research into practical solutions.

IHRI warmly congratulates the CUiHUB team on this milestone and looks forward to opportunities to work together in the future.

Congratulations to Dr Nittaya Phanuphak, Executive Director of IHRI, on winning the WHO South-East Asia Regional Award 2026 for Public Health Champions.

Over 25 years, she has pioneered HIV prevention and care models that are both innovative and equitable. Her work has helped shape Thailand's HIV policy and regional guidance, and shown how community-led approaches can accelerate the end of HIV as a public health threat.

We couldn't be prouder to celebrate this together.

For more than 25 years, Dr Phanuphak 🩺 has advanced HIV prevention, clinical research and service delivery and pioneered innovative and equitable HIV prevention and service delivery models.

She pioneered innovative and equitable prevention and service delivery models, including the Princess PrEP programme and Asia-Pacific region's first transgender-led clinic, Tangerine Community Health Center.

Her work has:

Some of the most powerful care doesn't come from institutions. It comes from the hands of those who've lived it.

Today in Amsterdam, the Netherlands, at The Hands that Heal: Community-led Shelters & Care, our Director of Implementation Research and Innovation, Rena Janamnuaysook, joined the conversation on how community-driven shelters and support services build something institutions so often can't: trust, cultural relevance, and care that meets people where they actually are.

Rooted in lived experience, these grassroots models center autonomy, collective decision-making, and solidarity, closing the gaps in dignity and responsiveness that formal systems too often leave open.

At IHRI, we believe the communities closest to the challenge are closest to the solution. Grateful to Rena for carrying that message forward. 🧡
