When a mother and her children are hit hard by drought, they need a place to lean on. A place that restores hope, and offers a shoulder to carry them through.

A mother walks for hours under the sun, carrying a child too weak to cry. She has watched her child grow thinner by the day, and fear has become part of every step of that journey. For many families in Wajid District, the walk ends at the Waaberi Stabilization Center.

Months of drought and ongoing conflict have pushed families into crisis. Pastoralist communities have been hit especially hard, and malnutrition rates have surged.

Mothers arrive here carrying children who are severely malnourished and fighting for their lives. They receive urgent medical care and nutrition support. The results speak for themselves: 98% of children treated at the center recover.

Children who arrive weak leave stronger, healthier, and full of hope.

The center is managed by Action Against Hunger USA in Somalia under the Caafimaad Plus Consortium, with funding from EU Civil Protection & Humanitarian Aid.

This work is only possible because of strong partnership and sustained support. As malnutrition levels remain high across Bakool, continued investment from donors and communities alike is what keeps this lifeline open. Because for these children, this is survival.

These are the people behind every number we report.

This World Humanitarian Day, under the theme , we celebrate the health workers and volunteers serving Somalia's communities every day. Through them, Caafimaad Plus Consortium has reached over 3 million children and families in 2025, often in the hardest circumstances imaginable, supported by EU Civil Protection & Humanitarian Aid and UK in Somalia.

They are not distant figures. They are our colleagues and neighbors, turning duty into action.

Yesterday was Day 1 of World Breastfeeding Week.

Caafimaad Plus Consortium joined the Ministry of Health- Somalia and other partners at the official launch in Mogadishu.

We stood for turning breastfeeding-friendly policies into everyday practice, in workplaces, in health facilities, in communities.

Because the first 1,000 days shape a child's future. And no mother should have to choose between going back to work and feeding her baby.

A missed vaccine is rarely one failure.

It's a family who couldn't reach a clinic before the rains cut off the road. A health worker who found a child a week too late. A dose that existed, but never arrived.

In South Central Somalia, closing that distance is not something any single actor can do alone, and that's the point.

Caafimaad Plus brings together health facilities and community outreach as one continuous system, not two separate efforts.

In clinics, children receive vaccines on schedule, delivered by trained staff with proper oversight. But for families displaced by conflict or drought, or living hours from the nearest facility, that clinic might as well not exist.

So the system meets them where they are.

Health workers go door to door, tracking who's missed a dose and who's never had one, catching gaps before they become emergencies, and referring children onward for treatment when a vaccine alone isn't enough.

This works because of who's at the table.

Juba Foundation's teams move fluidly between facility and community, the same staff who support clinical delivery are the ones walking the last mile to reach a family that couldn't otherwise be reached.

That's not a logistics detail. It's the difference between a programme that exists on paper and one that actually arrives at a door.

None of this happens without sustained investment.

Partnership, in immunisation, isn't a value statement. It's the operating model.

It's the only way coverage holds when the road floods, when a family moves twice in a season, when a single clinic can't be everywhere at once.

Where partners share the work, children don't fall through the space between systems.

We are expanding 𝐡𝐞𝐚𝐥𝐭𝐡𝐜𝐚𝐫𝐞 & 𝐧𝐮𝐭𝐫𝐢𝐭𝐢𝐨𝐧 𝐬𝐞𝐫𝐯𝐢𝐜𝐞𝐬 to remote villages in the 𝐁𝐚𝐲 𝐑𝐞𝐠𝐢𝐨𝐧 by training and equipping local Community Health Workers (CHWs)!

Through 𝐈𝐧𝐭𝐞𝐠𝐫𝐚𝐭𝐞𝐝 𝐂𝐨𝐦𝐦𝐮𝐧𝐢𝐭𝐲 𝐂𝐚𝐬𝐞 𝐌𝐚𝐧𝐚𝐠𝐞𝐦𝐞𝐧𝐭 (𝐢𝐂𝐂𝐌+), our frontline heroes are now:

Impact doesn't always look like big numbers.

Sometimes it looks like a health worker pulling up a chair for a mother who needed someone to show up. Unhurried. Human.

At Akara Health Centre in Luuq District, this is not a remarkable moment, it is a daily one. Mothers and children walk through the door and are met by Trocaire's dedicated teams, ready to listen, diagnose, and care.

This is what community-rooted healthcare looks like in Somalia. Not headlines. Not statistics. Just a health worker and a mother, in a room, with enough time and enough trust to make a difference.

In rural Somalia, a mother loaded her severely malnourished three-year-old onto a donkey cart and travelled 30 kilometres to reach the nearest hospital.

She didn't know where it was. She just kept moving.

Sustained health gains in Somalia depend on exactly this kind of coordination: consortium members aligned behind national frameworks like the National Transition Plan (NTP) and the Essential Package of Health Services (EPHS), and a Ministry that actively champions that alignment.

Across the villages and pastoralist communities of Galmudug, the consequences of prolonged drought are profound and compounding. Livestock lost. Harvests failed. Families displaced. And for thousands of people, the nearest health facility remains miles away, sometimes unreachable.

In humanitarian response, proximity is everything. When communities cannot come to care, care must go to them.

That is the principle driving our emergency health response in Galmudug.

Caafimaad Plus Consortium, through International Medical Corps, is on the ground delivering frontline health services to drought-affected communities, providing one-on-one consultations, outreach services, and direct service delivery to families who have been left most exposed by this crisis. Our teams are not waiting for patients to find them. They are going where the need is greatest.

This is what meaningful emergency health response looks like, showing up. Consultations for drought-displaced families delivered at their doorstep. Emergency health services brought directly to pastoralist communities across Galmudug. No waiting. No barriers. Just care.

Because distance and disaster should never determine who gets care.

The communities most forgotten by crisis response are almost always the ones carrying its heaviest burden. Closing that gap requires not just resources, it requires commitment, presence, and the willingness to go the last mile, every time. That is a responsibility we do not take lightly.

When nutrition caseloads spike, children don't get a second chance.

That's why preparedness isn't optional, it's everything.

Last week, Caafimaad Plus Consortium wrapped a five-day Community-based Management of Acute Malnutrition (CMAM) Surge Approach workshop, bringing together health facility workers and technical leads from across Banadir Region. The sessions were led by Concern Worldwide, which has championed this approach in Somalia since 2021.

Over five days, participants refreshed critical skills: surge monitoring, threshold setting, preparedness planning, and response actions. But it was more than a training. It was five years of implementation experience, the lessons, the setbacks, the breakthroughs, laid out on the table and examined honestly.

11 nutrition sites scaled up under the initiative. But the real achievement is systemic. The CMAM Surge Approach has built something Somalia's health system needs, the ability to see a crisis coming and act before it arrives.

Frontline health workers said it best: this approach supports timely decisions, strengthens facility-level preparedness, and keeps life-saving nutrition services running exactly when demand is highest.

That's anticipatory action. That's what protecting children's lives actually looks like.
