St Luke's Hospital - SLH

St Luke's Hospital - SLH A charity healthcare provider enriching lives in the communities since 1996 through clinical, social, and pastoral care.

St Luke’s Hospital, named after the patron saint of the medical profession, was the first hospital in Singapore dedicated to the care of older persons.

Today, we are a community care provider supporting needs through our care ecosystem, spanning inpatient, outpatient, home care and community-based programmes, and specialised areas of care in rehabilitation, dementia, wound and palliative care.

An Institution of a Public Character, we care for 2,500 inpatients and 5,000 outpatients each year, regardless of race, language or religion. Through our Clinical, Social, Pastoral (CSP) care model, we transform lives holistically beyond physical health.

Impactful and innovative in transforming community care, we were the first recipient of the President’s Award for Social Impact (2012), and the first community hospital to receive the National Healthcare Innovation and Productivity Medal (National University Hospital – St Luke’s Hospital Integrated Care Path, 2016).

We partnered the National University Health System (NUHS) to pioneer the Patient Appointment Consolidation (PAC) programme, which won the overall Excellence Champion Medal in the National Healthcare Innovation and Productivity Medals in 2017. In 2022, we were also awarded with the National Healthcare Innovation and Productivity Medal for the Community Response Team (CRT) project, and a joint-hospital Improving Mobility Via Exoskeletons (IMOVE) programme.

Through our concerted efforts to build a thriving workplace, we received the Great Place To Work® certification in 2023, a global standard for quantifying and benchmarking employee experience.

We were proud to partner with Fei Yue Families for Life for the recent 𝗟.𝗢.𝗩.𝗘. – 𝗙𝗮𝗺𝗶𝗹𝘆 𝗘𝘅𝗽𝗲𝗿𝗶𝗲𝗻𝘁𝗶𝗮𝗹 𝗟𝗲𝗮𝗿𝗻𝗶𝗻𝗴 event — a meaningful morning of learning, connection and togetherness.

At our community resource booth, we connected with members of the public and shared how our inpatient, outpatient, home care and community services support healthy living and active ageing. Through interactive activities, families also learnt simple and practical ways to practise L.O.V.E. — Listen, Observe, Validate and Encourage — in their everyday interactions.

As part of the ongoing National Family Festival, the event brought families together across generations for a time of learning, bonding and shared experiences. We are grateful for the opportunity to support this meaningful initiative and to work alongside our community partners in bringing wellness closer to the community.

Last weekend, we hosted our annual Palliative Memorial Service, bringing together families, caregivers and staff for a meaningful time of remembrance and reflection.

Through heartfelt caregiver sharing, a video montage of cherished moments, a staff song presentation and a remembrance observance, we reflected on the enduring impact of those we have journeyed with and celebrated the love they leave behind.

One particularly meaningful activity was the 𝘌𝘹𝘱𝘳𝘦𝘴𝘴𝘪𝘰𝘯 𝘰𝘧 𝘓𝘰𝘷𝘦 𝘢𝘯𝘥 𝘙𝘦𝘮𝘦𝘮𝘣𝘳𝘢𝘯𝘤𝘦, where guests penned personal messages to their loved ones before folding them into heart-shaped keepsakes — a simple yet poignant reminder that love endures beyond loss.

Reflecting on the journey of grief and healing, Josephine Koh, Assistant Director of Nursing (Outpatient Services) and Head of St Luke’s Community Wound Centre, shared, “Grief does not ask us to move on, but to gently move forward and keep living. The most beautiful way we can honour our loved ones is by caring for ourselves, because we are part of the legacy they spent a lifetime building.”

As a community of care, we remain committed to supporting caregivers and families beyond the end-of-life journey, offering comfort, connection and hope as they navigate life after loss.

A day of discovery, laughter and hands-on learning! ✨

From exploring the simulation room with our nurses, solving IT puzzles to stepping into the shoes of a lobby service ambassador, our little guests enjoyed a special behind-the-scenes glimpse into life at SLH.

Through our annual Kids@Work Day, our colleagues' children had the opportunity to experience firsthand the people, passion and purpose behind their parents' work. Along the way, they gained a deeper appreciation of community care and the many hands and hearts that come together to support healing and recovery.

Most importantly, it was a chance for families to spend quality time together, create lasting memories, and celebrate the meaningful work that makes a difference in the lives of others every day.

Did you know? In one of our earliest fundraising campaigns, donors could “buy a brick” – a symbolic contribution to the building fund that helped turned the vision of our hospital into reality.🧱

Over the years, the generosity of our donors has enabled us to grow, evolve and continue serving as a beacon of hope and healing for patients, families and the community.

As we celebrate our 30th anniversary this year, donors who support our cause can receive limited-edition collectibles inspired by the people, milestones and spirit of giving that have shaped our journey.

Join us in celebrating our legacy and shaping our future — and you might even bring home a brick of your own. 💙

We recently hosted representatives from Far East Organization (FEO) for a corporate learning visit to St Luke's Hospital.

During the visit, we shared about our care ecosystem and integrated Clinical-Social-Pastoral (CSP) care model, highlighting how we support patients and caregivers across different stages of their healthcare journey.

A key focus of the discussion was our work with seniors living at the Community Care Apartments in Bukit Batok, where we provide community healthcare services that empower residents to age well in place. We also exchanged perspectives on the importance of partnerships and cross-sector collaboration in ensuring continuity of care for patients beyond the hospital walls.

Thank you to the FEO team for the meaningful exchange of ideas. We look forward to deepening our partnership and continuing to work together to help seniors age well in the community.

We were honoured to partner with KIN for this year's Dementia Ministry Conference!

The event brought together more than 200 participants to explore how churches and communities can better support persons living with dementia and their caregivers.

As one of the invited speakers, our Chaplain and Lead of Care and Counselling, Winston Tay, spoke about the evolving nature of dementia and the unique challenges it presents at different stages. He also shared how we support patients and their families through our integrated care ecosystem, from our dedicated inpatient ward to our Community Inclusive Choir and homecare services.

We also had the opportunity to learn from keynote speaker Reverend Professor John Swinton, who offered thought-provoking reflections on hope, meaning and belonging through the lens of faith.

At the Discovery Fair, our team connected with participants and shared more about our services and commitment to compassionate, person-centred dementia care.

Together, we can strengthen support for persons living with dementia through greater understanding, compassion and collaboration.

What a wonderful afternoon of movement, fellowship and reflection at Raffles Marina Lighthouse!

As we celebrate our 30th anniversary, this visit was especially meaningful. Serving as a beacon that guides others through uncertainty, the lighthouse is a powerful reminder of our commitment to bringing hope, care and compassion to the patients and communities we serve.

The outing also brought colleagues together in the spirit of wellness, camaraderie and teamwork. Special shoutout to our creative teams in the photo challenge!

We were delighted to welcome Dr Hamid Razak 贺明医生 ஹமீது, Advisor to Jurong Spring Grassroots Organisations, and his Health Taskforce team to St Luke’s Hospital for a valuable exchange on advancing community health and wellness.

The visit provided an opportunity to share our mission and the work we do each day to care for patients, caregivers, and the wider community. We also had insightful discussions to better understand the needs of residents and explore how healthcare providers and grassroots partners can work together to strengthen support for the community.

As healthcare increasingly extends beyond hospital settings, strong partnerships within the community become ever more important. Through collaboration, we can better support residents in staying healthy, ageing well, and accessing care closer to home.

We look forward to future opportunities to work together in serving the community.
