MANILA, Philippines —Overseas Filipino workers (OFWs) have the right to choose where they undergo required medical examinations before working abroad, as the Department of Health (DOH) reiterated existing regulations prohibiting referral and decking practices among accredited medical clinics serving migrant workers. This came amid the Gulf Health Council's recent implementation of the WAFID program, which requires workers traveling to Gulf countries to undergo pre-departure medical checkups via the digital medical screening system.

In DOH Circular No. 2026-0566, dated October 5, 2026, the DOH emphasized that overseas employment applicants must undergo their Pre-Employment Medical Examination (PEME) only at DOH-accredited medical clinics and health facilities. OFWs are free to select their preferred DOH-accredited or DOH-operated facility, except for seafarers, whose medical examination costs are shouldered by their principals or shipowners.

The circular also prohibits medical clinics or groups of clinics from having exclusive control over medical examinations for workers bound for specific destination countries. This protects OFWs against arrangements that unnecessarily restrict their choice of medical service providers.

Among the practices expressly prohibited is the decking system, which requires applicants to register at a designated office before being assigned or referred to a medical clinic elsewhere. The DOH reiterated that these arrangements are not permitted under existing regulations.

The DMW welcomes the timely issuance of the DOH Circular because it supports the policy anchored in Republic Act No. 10022, which strengthened protections for Filipino migrant workers, and in DOH Administrative Order No. 2011-0015, as amended, which safeguards OFWs' right to choose their medical examination providers.

The circular also cited Joint Memorandum Circular No. 2015-0016, which abolished the monopoly of the Gulf Cooperation Council–Approved Medical Centers Association, Inc. (GAMCA) and its decking and referral system.

Medical clinics and testing centers found to be participating in prohibited referral or decking arrangements may face regulatory action by the DOH Health Facilities and Services Regulatory Bureau (HFSRB), including show-cause and cease-and-desist orders and suspension or revocation of their licenses.

The circular applies to all DOH-accredited Medical Facilities for Overseas Workers and Seafarers (MFOWS).

The DMW underscores the importance of ensuring that OFWs are informed of their rights throughout the recruitment and deployment process, including access to appropriate and accredited medical services without unnecessary restrictions.

For DMW, the policy reinforces a fundamental protection for overseas employment applicants: required medical examinations should not be used to limit their choices or to subject them to prohibited referral arrangements and charging of exorbitant medical fees. # # #
