Heart and Sole Africa Heart and Sole Africa (formerly Imidido Project) began in 2009 in Musanze, Rwanda. We serve more than 2,000 patients through 13 regional clinics.

Heart and Sole Africa provides free health services to those affected by podoconiosis. WHAT IS PODOCONIOSIS? Podoconiosis, meaning "dusty feet" is a non-infectious tropical disease. Also know as non-filarial elephantiasis, podoconiosis is a chronic condition that causes an asymmetrical swelling of the feet and lower legs. This swelling may lead to severe disability of the patient. Symptoms may include severe fatigue, a heavy swollen limb, discoloration of the skin, disfiguring nodule growth on the toes and feet. Podoconiosis affects 4 million people worldwide, and it is endemic in areas of tropical Africa, Central and South America and northwest India. In the Northern Province of Rwanda, the contraction of podoconiosis is caused by chronic exosure to high concentrations of silica found in the volcanic soil originating from the Virunga mountains. Many with podoconiosis in this region are subsistence farmers, and they work in their fields barefoot, therefore experiencing frequent exposure to the siica in the soil. There is currently no up-to-date data about Podoconiosis in Rwanda, but a study entitled "Endemic Elephantiasis of the Lower Legs in Rwanda and Burundi" conducted by Tropical and Geological Medicine in 1975, reported a prevalence of 7.10 per 1000 of lower leg elephantiasis in areas in Rwanda where alkali clay soils coexist with high altitude, high seasonal rainfall and low income. Footwork, the International Podoconiosis Initiative, is currently embarking on a research project that will map podoconiosis patients throughout Rwanda. Heart and Sole Africa will assist with this research. ABOUT RWANDA

Known as the "Land of a Thousand Hills" and "The Heart of Africa", Rwanda is a small, mountainous country in East Africa bordering Uganda, Tanzania, Burundi, and The Democratic Republic of Congo. Rwanda is home to more than 11 million people, making it the most densely populated country in Africa. Rwanda is primarily an economy of subsistence farmers but continues to take steps to shift forward to a knowledge based economy. Although international dialogue often recounts the 1994 Genocide, Rwanda has embarked on a steady trajectory of growth, reconciliation, peace, and promise throughout the past decade. Heart and Sole Africa's main office is located in Musanze. This area was previously known as Ruhengeri, and it lies in the middle of the famous of Virunga Mountain Range. The mountains, Karisimbi, Visoke, Mgahinga, Muhavura, and Nyirahongo are visible on the horizon daily as the Heart and Sole Africa team works diligently to help those in need. Satellite clinics exist in Kinoni and Nyange. Podoconiosis is a disease that manifests in this region because of its unique topography. Silica deposits filter up into the soil in these volcanic regions. In this predominantly agrarian economy, this has a profound impact. In a country where the doctor to patient ratio exceeds one to 16,046, there lacks specialized healthcare providers. Heart and Sole Africa operates all throughout remote regions in the Northern Province, even extending across the border into Uganda.

🤝 Turning knowledge into care

After three days of training in Ngozi, , healthcare workers put their new knowledge into practice during a supervised clinical session at Gitamo Health Centre, Tangara Health District. Working alongside experienced facilitators, they assessed, staged, and cared for people living with suspected podoconiosis.

For many patients, this was the first time they had received care specifically for . They were introduced to essential self-care practices, including foot soaking, washing, skin care, and the importance of protective footwear. Each confirmed patient also received a home-care kit and a follow-up appointment to support continued care.

These practical sessions demonstrate how training can be translated into patient care while strengthening local capacity. They also reinforce the importance of simple, consistent self-care in improving the lives of people affected by podoconiosis.

Every step taken today helps build the foundation for better care tomorrow.

The mission was funded by Footwork and implemented by the UGHE Centre for One Health through the The Resilient and Sustainable Health Systems Approach for Podoconiosis Elimination (RESHAPE) initiative, at the invitation of the Ministry of Health of Burundi and with the support of WHO AFRO-ESPEN. HASA was represented by Jeanne Uwizeyimana, Program Director.

Podo is caused by barefoot exposure to irritant volcanic soils. Heart and Sole Africa is dedicated to caring for those suffering from this condition and are focusing on the next generation with our prevention boot campaign. We have established 13 clinics across Rwanda and hope to add 7 more clinics this year. We could not have done this without our donors. Thank you! I want to send a special thank you to all our monthly donors. If you would like to become a monthly donor at any level, you can do that securely at www.heartandsoleafrica.org or at Mason Bank, Mason Texas. You can be part of the team eliminating Podo in our lifetime!

The 24 hour journey across the globe has landed me back home in Texas, to my delight, and into the loving home of Jack and Karen Grosskopf to rest up for my drive back to Mason. The important work of Heart and Sole Africa continues on a daily basis with the Rwandan team, in our efforts to improve the lives of those with podoconiosis, freeing them from being marginalized and stigmatized. Thank you for your support and for being part of a team trying to eliminate this neglected tropical disease. “Let freedom ring!”

This group has been very engaged in Podo education this week and includes health center directors, nurses, and physiotherapists across the northern part of Rwanda. Training will commence next week for the southern clinic providers. Facilitating the education is the HASA staff and RBC from the Ministry of Health. This is a collaborative effort to integrate Podo services to all those suffering from this debilitating condition while preventing new cases.

I have been in Rwanda for 30 days and it has not rained at all which is unusual…. It is the dry season, but very dry this month. Water catchment systems are out of water and our patients are now carrying water for foot soaking. We are continuing our Podo preventative education, boot distribution activity, and I was very happy to see this woman wearing her boots!

Today we began a Podoconiosis management training for Rwandan healthcare providers that work in health clinics across Rwanda. It is a continuous effort by HASA and the Rwandan Ministry of Health to integrate Podo services into the existing health system. Many thanks to the Izumi Foundation, Footwork and HASA donors for making this possible!

This past weekend I spent some time at Lake Burera with friends. Rwanda is a beautiful country and you can see the Virunga Volcanic Mountain range across the lake. The fertile soil here is what causes Podo… barefoot exposure. The last picture is Lucy the milk cow who is very social. I was resting in the hammock when she came up to say hello 😆.

The dirt road to my friend Alberto’s La Locanda BNB was turned into a movie set while I was here in Rwanda. 30 German crew were here filming a show for the TV series in Germany called the Love Boat that has been running for many years. You ask?…. Where is the boat? The story line is taking someone on the boat back to their country. It will be out next April. It was interesting to watch this unfold from the set design to filming, and it took 2 years of planning to make it happen.
