🧠 What are Statins?
Statins are lipid-lowering drugs that inhibit HMG-CoA reductase → reducing LDL cholesterol and preventing atherosclerotic cardiovascular disease (ASCVD).

⚠️ When to Start Statins? (Common Indications)

1️⃣ Established ASCVD
• Previous MI, stroke, or peripheral arterial disease
→ Start high-intensity statin

2️⃣ LDL ≥ 190 mg/dL
• Suggests familial hypercholesterolemia
→ Start high-intensity statin

3️⃣ Diabetes Mellitus (Age 40–75)
• LDL ≥ 70 mg/dL
→ Start moderate-intensity statin

4️⃣ High 10-Year ASCVD Risk
• Age 40–75
• LDL ≥ 70 mg/dL
→ Start statin depending on risk level

💊 Statin Intensity

🔴 High-Intensity (↓ LDL ≥50%)
• Atorvastatin 40–80 mg
• Rosuvastatin 20–40 mg

🟠 Moderate-Intensity (↓ LDL 30–49%)
• Atorvastatin 10–20 mg
• Rosuvastatin 5–10 mg
• Simvastatin 20–40 mg

🧪 Baseline Tests Before Starting
• Lipid profile
• ALT (liver enzyme)
• Consider CK if risk of myopathy

⚠️ Important Precautions
• Avoid in active liver disease
• Contraindicated in pregnancy
• Watch for myalgia or muscle weakness

Follow Medical Forum KPK Acute Stroke – Management in the First 24 Hours

The first 24 hours after stroke onset are critical for preventing brain damage and improving patient outcomes. Rapid assessment and timely treatment are essential.

🧠 1️⃣ Immediate Assessment (ABC First)
• Airway: Ensure airway is patent
• Breathing: Give oxygen if SpO₂ < 94%
• Circulation: Check pulse, blood pressure, IV access
• Check blood glucose immediately (hypoglycemia can mimic stroke)

⏱️ 2️⃣ Determine Time of Onset
• Ask: “When was the patient last seen normal?”
• This determines eligibility for thrombolysis

---

🧪 3️⃣ Initial Investigations
• Non-contrast CT scan of brain (urgent)
• Blood glucose
• CBC, electrolytes
• Coagulation profile (PT/INR)
• ECG ± cardiac enzymes

Purpose: Differentiate ischemic vs hemorrhagic stroke

💉 4️⃣ Thrombolysis (If Ischemic Stroke)
• Alteplase (tPA) if within 4.5 hours of symptom onset
• Dose: 0.9 mg/kg IV (max 90 mg)
• 10% bolus
• 90% infusion over 60 minutes

⚠️ Contraindicated in hemorrhage or high bleeding risk.

🩸 5️⃣ Blood Pressure Management
• If candidate for thrombolysis → keep BP < 185/110 mmHg
• If NOT receiving thrombolysis → treat only if BP > 220/120 mmHg

Common drugs:
• Labetalol IV
• Nicardipine infusion

💊 6️⃣ Antiplatelet Therapy
• If ischemic stroke and no thrombolysis:
→ Start Aspirin 160–325 mg within 24 hours

⚠️ If tPA given, delay aspirin for 24 hours

🧊 7️⃣ Supportive Care
• Maintain normoglycemia
• Control fever
• Maintain adequate hydration
• Elevate head of bed ~30°

---

🧪 8️⃣ Monitor for Complications
• Cerebral edema
• Aspiration pneumonia
• Seizures
• Increased intracranial pressure

📊 9️⃣ Early Stroke Unit Care
Patients managed in a dedicated stroke unit have significantly better outcomes and lower mortality.

یہ ایک خاتون کا بایاں ہاتھ ہے جس کا انگوٹھا بیماری کی وجہ سے کاٹنا(Amputation) پڑا۔ اس کے علاوہ اس کا دایاں بازو بھی کاٹا ہوا (Amputated) تھا۔ اور اب وہ سانس کے مسئلے (ILD and Pulmonary hypertension) کے ساتھ داخل ہوئی تھی۔

A female patient with ITP (Immune Thrombocytopenia)
ایک نوجوان مریضہ کو ایک ہفتہ پہلے مسوڑوھوں سے خون آنا شروع ہوا (Gum bleeding)، ٹانگوں پہ نشان آنا شروع ہو گئے (جیسے فوٹو میں دکھایا گیا ہے، Petechiae and purpura)، آنکھوں میں بھی خون کے نشان تھے (Subconjunctival hemorrhage) جیسا کہ فوٹو میں دیکھا جا سکتا ہے۔ مزید ہسٹری پہ انہوں نے بتایا کہ انہیں کچھ عرصے سے ماہواری بھی زیادہ آ رہی ہے یا دو بار آجاتی ہے (menorrrhagia) ۔ اس بیماری کو ہم Immune Thrombocytopenia یا ITP کہتے ہیں!

Testicular torsion happens when a testicle twists on its spermatic cord.
