4. Barangay Residency Certificate ng isa sa mga magulang ng aplikante/ estudyante na nagpapatunay ng tagal ng paninirahan sa Taguig na hindi bababa sa tatlong (3) taon.
5. Photocopy ng Birth Certificate ng aplikante/estudyante
6. (Optional - ngunit mas mapapabilis ang proseso kung dala ito)
