For those who are interested to join us the contest, this is open for everyone! Costumes with too wet props that may cause the floor to be too slippery are not encouraged. Please make it as dry as possible. Just prepare a Php 50 for your registration. Registration will start at 7:00 PM

What: PhilSys Registration (National ID)
When: August 30, 2024 start at 8am
Where: Barangay Poblacion 4 GYM

1. for AGE 1-4 years old
Requirements:
A. ORIGINAL BIRTH CERTIFICATE or PSA copy
B. Dapat naay National ID ang Mama or Papa nga kuyog during registration kay kinanglan ang iyang PCN nga makita sa NATIONAL ID/EPHIL ID og kinahanglan kuyog pud ang Mama or Papa kay siya ang e Biometric.
C. Kung Guardian, Lolo, Lola, Aunt/Uncle ang mokuyog, dapat naay National ID na og Authorization gekan sa mama or papa.

2. for age 5-17 years old
Requirements:
A. ORIGINAL COPY BIRTH CERT, LCR OR PSA
B. School Enrollment Form kung walay birth certificate ang magpa register

3. for age 18-above
Requirements:
ORIGINAL COPY BIRTH CERT, LCR OR PSA
ORIGINAL COPY Marriage Cert kung ang valid ID kay single pa nya namenyo na.

4. Para pod sa mga wala pa jud naka dawat sa ilang mga NATIONAL ID palihog dal-a ang inyong TRANSACTION SLIP

5. Para pod sa mga NAWALA nga TRANSACTION SLIP palihog og adto pod para ma request ang inyong TRANSACTION REFERENCE NUMBER.

6. Para pod sa mga NANGAPANAS/NAPAKSI nga picture sa ilang NATIONAL ID palihog pwede nato e surrender dal-a lang palihog ang NATIONAL ID og Envelope kauban ang papel nga gisudlan sa National ID card.

7. Para pod sa mga naay sayop sa ilang NATIONAL ID palihog adto pod mo para makuhaan mo daan og detalye kay sa UPDATING puhon pwede na ma correct ang sayop.
