๐จ๐ซ ๐๐๐ญ๐ฎ๐ซ๐ง๐๐๐ฌ (enrolled in college, but stopped studying in BukSU before 2018): Certification from the Registrar to be requested via this form (link will be provided upon evaluation)

1. Personal email address (Gmail account)
