Basic Requirements
✅Personal appearance
✅Accomplished passport application form
✅PSA-issued Certificate of Live Birth (original & photocopy)
👉For married females using spouse’s surname
✅PSA-issued Certificate of Marriage (original & photocopy)
✅Valid government-issued ID (original & photocopy)
✅Local Civil Registrar (LCR) copy if PSA copy is unclear
👉For late registered birth certificate – provide any of the following:
✅One (1) additional primary government-issued valid ID; OR
✅Any two (2) of these:
NBI Clearance
School records (Form 137-A, TOR, Diploma)
Service Record (if government employee)
PhilHealth MDR

✅Current ePassport (original & photocopy of data page)
✅PSA-issued supporting documents for name change, if applicable:
Certificate of Marriage
Annotated Birth or Marriage Certificate
Court Order or Certificate of Death (for widows)
Valid ID (original & photocopy)
👉For Non-ePassport (Brown/Green/Maroon):
✅PSA Birth Certificate (original & photocopy)
✅PSA Marriage Certificate (if applicable)
✅LCR copy if PSA copy unclear
✅Acceptable ID with photocopy
👉 FOR MINOR – NEW OR RENEWAL APPLICATION
Basic Requirements
✅Accomplished application form
✅Personal appearance of minor and parent/authorized adult companion
✅PSA-issued Birth Certificate (original & photocopy)
✅School ID (for ages 8+)
✅Valid ID of accompanying parent/guardian
✅Current ePassport (for renewal)
✅Supporting Documents (depending on case):
✅If no PSA Birth Certificate yet → CTC from LCR or Report of Birth
✅If late registered → School record or baby book
✅If parents married → Valid ID/passport of both parents
✅If parents not married → Mother must accompany or provide SPA
✅If minor not with parents → Special Power of Attorney designating adult companion + parent’s valid ID copies
✅If mother deceased → Father’s ID + Death Certificate of mother
✅If both parents deceased → Guardian’s ID + Court Order of Guardianship
