Hello Parkway Village HOA Seniors,

Greetings!

We were required by Kap. Elenita “Kap Ine” De Jesus, being the head of Barangay Apolonio Samson Seniors Citizens Organization (BASSCO), to register with the National Commission of Senior Citizens and with the Senior Alliance of District 6 of Quezon City so we can get benefits whatever is allotted for senior citizen from the government.

In view of this, attached herewith is an application form to be filled up by all seniors in our Parkway Chapter, we can submit the necessary requirements to oyr barangay Office.

Please submit also a copy of your Senior Citizen ID and your latest photo of any size, whatever is available for as long as it is clear

Thank you very much for your compliance. We will collect the same from you on or before April 12, 2025**, or if it is ready for collection, and / or for whatever queries of clarification, please call or message our Secretary Ellen R. Leano with her mobile no. 0917 532 4760
