📌 Important Reminders for Game-Changers
To make your Loyalty Card Plus application smooth and hassle-free, please take note of the following:
✅ What to Bring
2 Valid IDs
PSA Birth Certificate and/or Marriage Contract
Exact amount of Php 125 (Cash Only) for the card fee
⚠️ If You Have Info Discrepancies in Pag-IBIG Records
Common issues include:
-Mother’s Maiden Name
-Special characters/suffixes in your name (e.g., ñ, Jr., Sr., II, III)
-Gender, Citizenship, Marital Status
To avoid delays, please bring:
-📄 1 photocopy of your PSA Birth Certificate
-🪪 1 photocopy of a valid ID
-💍 1 photocopy of your Marriage Contract (if applicable)
📝 If You Need to Update or Correct Your Info
You may be asked to fill out a Member’s Change of Information Form (MCIF) onsite. Depending on the correction, you might also need:
Court Order (for legal name/marital status changes)
CENOMAR (for marital status corrections)
Death Certificate (for widowed status)
Proof of employment or address change
🆔 Accepted Valid IDs
Pag-IBIG accepts a wide range of IDs, including:
-Passport, Driver’s License, PRC ID
-SSS/GSIS Cards, PhilSys ID
-Company ID (regulated by BSP, SEC, or IC)
-Barangay ID, Postal ID, Voter’s ID
-Senior Citizen Card, Seaman’s Book, OFW ID
-DSWD or NCWPD Certifications, IBP ID
