AWPI HISTORY

One of the resolutions in the consultation done among artist-leaders who attended the Ikatlong Tagpo: 3rd National Theater Festival held at the Cultural Center of the Philippines (CCP) in 2004 was to address the welfare needs of the artists, especially those who, since the Martial law Days, have unselfishly dedicated their lives to arts and cultural work, and have contributed immense

ly to the toppling of the dictator and to liberating the Filipino people from an oppressive administration. Many of these artists and cultural workers are now in their senior years, with many of them not prepared for old age, when they have already become sickly, with no savings, no house of their own, no social security benefits, no retirement benefits. In the succeeding years, 2005-2007, it became almost a regular occurrence that the members of the artistic community would be informed that a colleague has a debilitating disease, cancer, diabetes, hypertension, aneurysm, heart enlargement, blocked arteries, or that another artist passed away leaving huge hospital bills and with no funds for burial. The artistic community, being generally, a compassionate, caring community, would "pass the hat" to collect donations. After a series of fund collection and fund raising for the sick, the dying and the dead, a group of concerned artists decided to form the ARTISTS WELFARE PROJECT INC (AWPI) in 2007. Its main reason for being is precisely to spearhead a campaign and implement a program to address the welfare needs of the old members of the artistic community, while also teaching and preparing the younger members of the artists' sector to take care of their present and future needs.

HOW TO USE AWPI HMO - MAXICARE CARD

AWPI-MAXICARE ID
1. We can use the coverage by March 27, 2026, however since the lists are still populating in the systems and databases, best to have check ups by end of April. As for Emergency cases, please pm Jennifer Lee Bonto asap for assistance.
2. The ID will arrive by April. Jenny will pm you if the cards are ready for pick up. Will message pick up address.

At Doctorโ€™s Clinics
1. When at clinic or hospital, show the card and any valid ID to the HMO Desk Officer. As Guard where the HMO desk is at.
2. Wait for the officer to give you a LOA document
3. Show document to doctor.
4. If lab tests are needed, go back to HMO Desk Officer and get another LOA for Labs.

At the EMERGENCY ROOM:
1. Show Maxicare ID or any valid ID to the Nurse. Remember to mention you are AWPI - Artists Welfare Project Inc member. Follow instructions after.

When being admitted
1. Show admissions your Maxicare Card. Follow instructions of nurse after.

IMPORTANT INFO On Pre Existing Disease:

A disease is considered to be Pre-existing (PEC) if any of the following cases are present (with or without the patientโ€™s knowledge): -such illness or injury was in any way evident to the Insured before the effective date of the insured's coverage. - any professional advise or treatment was obtained prior to the effective date of the Insured's coverage - the illness or injury can be clinically determined to have started prior to the effective date of the insured's coverage.

INCLUSIONS

- patient has an option to upgrade room
- patient covers room fees difference
2. Doctors' fees, laboratory fees, hospitalization fees
3. Basic dental services (1 dental cleaning )
4. Annual Physical Exam (APE)

EXCLUSIONS:
1. Dependents are not included.
2. St Lukes BGC, QC , Medical City Ortigas, Cardinal Santos and Asian Hospital, are not included in the list of covered hospitals UNLSS you enrolled it with Jenny
3. Mental Health Services
