Why Is Breastmilk Still Safe During Bacterial Mastitis?

I often get questions from moms if it's still safe to breastfeed or give expressed breastmilk even if they have on going mastitis or breast abscess.

Even though bacteria are present in the breast tissue, the breastmilk itself remains safe because:

1. The infection is in the tissue, not the milk
Mastitis is inflammation/infection of the breast tissue, not an infection of the milk ducts.
The milk is not “infected.” It simply has to pass through inflamed areas to exit the breast.

2. Breastmilk contains strong natural immune protection
Breastmilk is rich in:
Secretory IgA
Lactoferrin
Lysozyme
Leukocytes
These components actually kill or neutralize bacteria, making the milk safe for the baby.

3. The baby is already colonized with the same skin bacteria
Most mastitis-causing bacteria are common skin flora (Staph aureus, Staph epidermidis).
These bacteria are already on the mother’s and baby’s skin, so ingesting breastmilk does not pose new risk.

4. Milk stasis—not the bacteria—is the main cause
Mastitis begins with milk stasis and inflammation, and only sometimes progresses to bacterial infection.
Continuing breastfeeding directly treats the underlying problem.

5. No evidence of harm
ABM states there is no evidence that infants are harmed by consuming milk from a breast with mastitis—and decades of clinical practice support this.

🥛 In the simplest terms:
The infection is in the breast tissue, not the milk.
Breastmilk has built-in antibacterial protection.
So it stays safe for the baby—even during mastitis.
