Lived Names and Preferred Pronouns Recognition and Data Collection Form

𝗔𝗰𝗰𝗲𝘀𝘀 𝘁𝗵𝗲 𝗳𝗼𝗿𝗺𝘀 𝗵𝗲𝗿𝗲:

In line with its commitment to advancing students’ rights, welfare, dignity, and inclusivity across the PUP System, the Alyansa ng Nagkakaisang Konseho ng Politeknikong Unibersidad ng Pilipinas (ANAK PUP) together with the PUP Office of the Student Regent (PUP OSR) is spearheading the collection and verification of information from PUP graduating students who wish to have their lived names and preferred pronouns recognized during graduation and other recognition ceremonies.

Students who intend to use their lived names are encouraged to accomplish the form to ensure that their preferred names and other necessary information are properly recorded and communicated to the concerned Offices and Student Councils. All information collected through the form will be used solely for coordination, verification, and the proper implementation of lived name recognition during the graduation rites.

Students who fail to accomplish the online form may coordinate with their respective Student Councils for on-site data collection and verification during the graduation rehearsals.
